胸腔镜手术治疗高龄气胸患者的临床体会

摘　　要：目的探讨胸腔镜手术治疗高龄自发性气胸的安全性。方法回顾性分析2008年6月~2016年6月26例70岁以上因自发性气胸行全胸腔镜手术的临床资料。结果手术时间(62±28)min,出血量(35±15)ml,无中转开胸。术后胸腔引流时间(4.8±1.8)d。术后并发症4例,1例因呼吸衰竭于围手术期死亡。25例随访(38±16)月,无复发。结论胸腔镜治疗高龄自发性气胸安全、有效。Objective To study the safety of video-assisted thoracic surgery（ VATS） in the treatment for spontaneous pneumothorax in elderly patients. Methods Between June 2008 and June 2016,26 patients over the age of 70 years old received thoracoscopic surgery treatment for spontaneous pneumothorax. The clinical data were retrospectively observed. Results The average operation time was（ 62 ± 28） min and the average intraoperative blood loss was（ 35 ± 15） ml. No conversion to thoracotomy was required. The postoperative drainage period was（ 4. 8 ± 1. 8） days. Four cases suffered from postoperative complications,and perioperative death occurred in one patient due to respiratory failure. During a follow-up of（ 38 ± 16） months in 25 patients,no recurrence was observed. Conclusion VATS is a safe and effective procedure in the treatment of spontaneous pneumothorax in elderly patients.
