2型糖尿病患者并发脑卒中的前瞻性研究

摘　　要：目的了解2型糖尿病(T2DM)患者脑卒中发生率及影响因素,为预防T2DM患者发生脑卒中提供依据。方法选取浙江省慢性病监测信息管理系统2011年报告的余杭区T2DM患者1 129例,并进行随访观察至2014年10月,记录脑卒中发生情况,并分析影响因素。结果随访1 129例T2DM患者,发生脑卒中59例,发生率为5.22%,年发病率为1.56%,其中缺血性脑卒中56例(94.92%)。多因素Logistic回归分析显示,年龄≥65岁、高血压为T2DM患者并发脑卒中的危险因素,65岁~及75岁~的T2DM患者并发脑卒中的风险分别是45岁~的3.38倍(OR=3.38,95%CI:1.39~8.24)和7.77倍(OR=7.77,95%CI:2.14~28.24),合并高血压的T2DM患者并发脑卒中的风险为T2DM患者的10.92倍(OR=10.92,95%CI:5.94~20.09)。结论高龄、合并高血压是T2DM患者并发脑卒中的危险因素,应加强血糖、血压控制以预防脑卒中的发生。Objective To learn the stroke occurrence status and the influencing factors of the patients with type 2 diabetes,and to provide the basis for the prevention of stroke. Methods Patients with type 2 diabetes which were reported in 2011 year from the system of chronic disease monitoring information management of Zhejiang Province were selected and followed- up until October in 2014. Stroke occurrence were recorded and the influencing factors were analyzed. Results The incidence rate was 5. 22%（ 59 /1 129）,and the annual incidence was 1. 56%. Fifty six cases（ 94. 92%） were ischemic stroke. Logistic regression analysis showed that older than 65 and hypertension were risk factors for type 2 diabetes complicated with stroke. The group of 65- and 75- were associated with higher risk of stroke than 45-（ OR = 3. 38,95% CI： 1. 39- 8. 24; OR = 7. 77,2. 14- 28. 24）. Hypertensionin were the influencing factors of stroke among patients with type 2 diabetes（ OR = 10. 92,95% CI： 5. 94- 20. 09）. Conclusion Older age and hypertension were risk factors for type 2 diabetes complicated with stroke. Therefore effective control blood pressure among older patients with type 2 diabetes should be strengthed to prevent stroke.
