摘 要:[目的]调查消化道肿瘤化疗病人营养状况、生活质量在6个化疗周期中的变化趋势及相互关系。[方法]选取2013年10月—2014年8月南京市某三级甲等医院肿瘤科入院行初次化疗的73例消化道肿瘤病人作为研究对象。病人入院24h内及每个化疗周期采用改良版病人主观全面评价量表(abPG-SGA)进行营养评价,采用欧洲疾病研究与治疗组织生活质量测定量表(EORTC QLQ-C30)评定病人生活质量。[结果]6个化疗周期中病人营养不良发生率由50.79%上升至69.84%。辅助化疗病人的社会功能随化疗周期的延长呈现逐渐下降趋势,姑息化疗病人的躯体功能、角色功能、情绪功能、社会功能及总体健康状况随化疗周期的延长呈现逐渐下降趋势,化疗病人疲乏、恶心呕吐、食欲丧失及经济困难随化疗周期的延长呈现逐渐加重趋势。6个化疗周期中,病人营养评分与生活质量中的躯体功能、角色功能、情绪功能、认知功能、总体健康状况均呈负相关(P<0.05)。病人营养评分与生活质量中的疲乏、恶心呕吐、食欲丧失均呈正相关(P<0.05)。[结论]消化道肿瘤病人化疗期间随着营养状况的下降,生活质量变差。提示医护人员应及时对消化道肿瘤化疗病人的营养状况进行评估,合理进行营养干预,从而提高病人生活质量。Objective:To investigate changing trends and mutual relations of nutritional status and quality of life of patients with gastrointestinal cancer undergoing chemotherapy in 6chemotherapy cycles.Methods:A total of73 cases of gastrointestinal cancer patients received initial chemotherapy in department of tumor in a third grade A hospital in Nanjing city from October 2013 to August 2014 were selected as the research objects.Patient's quality of life was assessed by using the(European Organization for Research and Treatment of cancer Qualityof-Life Questionnaire C30,EORTC QLQ-C30).the nutritional status was evaluated by using(Abridged Scored Patient-Generated Subjective Global Assessment,abPG-SGA)within 24 hours of admission and each cycle of chemotherapy.Results:The incidence of malnutrition in patients during 6chemotherapy cycles increased from50.79% to 69.84%.Social function of patients with adjuvant chemotherapy showed a gradual downward trend with the prolong of chemotherapy cycle;the physical function,role function,emotional function,social function and overall health status of patients with palliative chemotherapy showed a gradual decline trend with the extension of the chemotherapy cycle.fatigue,nausea and vomiting,loss of appetite and the economic difficulties in chemotherapy patients showed a gradual aggravating trend with the prolong of chemotherapy cycles.During the6 chemotherapy cycles,patient's nutritional score was negatively correlated with the physical function,role function,emotional function,cognitive function,and overall health status in quality of life(P〈0.05).patients' nutritianal score was postively correlated with the fatigue,hausea and vomitings and loss of appetite in quality of life(P〈0.05).Conclusion:Quality of life of patients with qastrointestinal cancer during chemotherapy showed a gradual decline trend with decrease of nutritional status.It is suggested that medical staff should evaluate nutritional status of patients with gastrointestinal cancer underg
