经脐“︺”形切口成人阑尾切除术46例报告

摘　　要：目的探讨经脐＂︺＂形切口阑尾切除术治疗成人阑尾炎的安全性和可行性。方法回顾性分析46例急性阑尾炎患者行经脐＂︺＂形切口成人阑尾切除术的临床资料,在连续硬膜外麻醉下行经脐＂︺＂形切口阑尾切除术,术中采用自动照明拉钩（专利号：ZL 2015 2 0377761.9）。结果46例患者,3例因患者肥胖,术中不同意延长脐下皮肤切口,改行腹腔镜阑尾切除术,其余43例行经脐＂︺＂形切口阑尾切除术均获成功。手术时间平均42（25~90）min。术后平均住院4.2（3~7）d。30例切口轻微红肿,11例脐窝有积液,2例切口感染经局部换药治愈。所有切口愈合后外观满意,无粘连性肠梗阻、阑尾残株炎、粪漏、腹腔脓肿等并发症发生。结论经脐＂︺＂形切口阑尾切除术治疗成人阑尾炎安全有效,适应证宽,损伤小,费用低,且美容效果好,值得临床推广应用。Objective To investigate the feasibility and safety of open appendectomy by transumbilical＂︺＂incision in adult appendicitis patients. Methods A retrospective study was carried out in 46 adult patients with acute appendicitis. A transumbilical ＂︺＂-like incision was used in open appendectomy under continuous epidural anesthesia. And the retractor-illuminator was used for both abdominal muscle traction and surgery field illumination. Results All the operations were successfully achieved except 3 cases converted to laparoscopic surgery. The operation time was 25 to 90 minutes,with an average of 42 minutes. The hospitalization time was 3 to 7 days,with an average of 4. 2 days. There were 30 cases got the incisions slightly red and swollen,11 cases got umbilical fossa hydrops,and 2 cases got the wound infected. And all patients recovered well with no serious complication. Conclusion The transumbilical ＂︺＂-like incision in adults appendectomy is safe and effective with less damage,low cost and good cosmetic results.
