摘　　要：目的观察杨氏3A+疗法"膝五针"治疗髌下脂肪垫损伤的临床疗效及安全性。方法将60例髌下脂肪垫损伤者随机均分为治疗组(埋线针刀)和对照组(针灸组)2组。在治疗前后以VAS评分和膝关节活动度为观察指标,对2组的复发率和有效率进行统计学分析。结果治疗组总有效率86.67%,对照组总有效率63.33%,差异具有统计学意义(P<0.05);治疗前,2组VAS评分比较无统计学差异(P>0.05),与治疗前相比,2组VAS评分明显降低(P<0.01),2组间比较差异具有统计学意义(P<0.05);随访1年,治疗组复发率20%,对照组48.15%,差异具有统计学意义(P<0.05)。结论埋线针刀能够显著改善髌下脂肪垫损伤者疼痛症状,杨氏3A+疗法"膝五针"治疗髌下脂肪垫损伤安全有效,值得临床推广。Objective To observe the clinical effect and safety of Yang＇s 3A＋ therapy at ＂ Xiwuzhen＂ in the treatment of injury of infrapatellar fat pad. Methods 60 cases of patients with injury of infrapatellar fat pad were randomly divided into two groups. 30 cases in treatment group were treated with catgut embedding needle knife, and 30 cases in the control group were treated with acupuncture. VAS score and knee joint activity were observed in two groups before and after treatment. The recurrence rate and effective rate were analyzed. Results The total effective rate of the treatment group and the control group was 86.67% and 63.33%, respectively, and the difference was statistically significant （P〈0.05） . The VAS score between two groups had no significant differences before treatment （P〉0.05） , and VAS scores of the two groups after treatment decreased significantly when comparing with that before treatment （P〈0.01） , and there was significant difference between two groups （P〈0.05） . The recurrence rate of the treatment group and the control group was 20% and 48.15%, respectively, after 1 year of following-up, and the difference was statistically significant （P〈0.05） . Conclusion Catgut embedding needle knife can significantly improve the pain of injury of infrapatellar fat pad, Yang＇s 3A ＋ therapy at ＂ Xiwuzhen＂ in the treatment of injury of infrapatellar fat pad was safe and effective, and it is worthy of clinical reference and promotion.
