摘  要:目的:对比研究CT与MRI对不同类型外伤性寰枢椎骨折的诊断价值。方法:回顾我院70例高度怀疑外伤性寰枢椎骨折患者的CT及MRI影像资料,评估CT与MRI检查对外伤性寰枢椎骨折中不同骨折类型的检出结果及显示情况。结果:在70例患者中CT发现寰枢椎骨折61例,其中屈曲型34例,伸展型9例,垂直型12例,旋转侧屈型6例;MRI发现寰枢椎骨折58例,其中屈曲型36例,伸展型3例,垂直型11例,旋转侧屈型8例。结论:对外伤性寰枢椎骨折的总体检出率CT高于MRI,不同受力机制的外伤性寰枢椎骨折CT与MRI的检出率存在着较大的差异。伸展型骨折CT的检出率明显高于MRI,屈曲型骨折和旋转侧屈型骨折MRI的检出率略高于CT,垂直型骨折CT与MRI的检出率无明显差异。Objective: To compare the diagnostic value of CT and MRI for different types of traumatic atlantoaxial vertebral fractures.Methods: CT and MRI imaging data of 70 cases with highly suspected traumatic atlantoaxial vertebral fractures were reviewed,and the results and display of CT and MRI examination for different fracture types of traumatic atlanto-axial vertebrae were evaluated.Results: Among the 70 cases,61 cases of atlanto-axial vertebral fractures were found by CT,including 34 cases of flexion type,9 stretch type,12 verticaltype and 6 of rotary lateral flexion type.Fifty-eight cases of atlanto-axial vertebral fracture were found by MRI,including 36 cases of flexion type,3 stretch type,11 vertical type and 8 of rotary lateral flexion type.Conclusion: Overall detection rate of CT for traumatic atlanto-axial vertebral fractures is higher than that of MRI,and there is a significant difference of detection rate between CT and MRI for traumatic atlanto-axial vertebral fractures of different stress mechanism.Stretch fracture detection rate of CT is obviously higher than that of MRI,and flexion fracture and rotary lateral flexion fracture detection rate of MRI is slightly higher than that of CT.No significant difference of vertical fracture detection rate is found between CT and MRI.
