腔镜下老年腹股沟疝修补术中置引流管的临床分析

摘　　要：目的：探索腔镜下老年腹股沟疝术后腹股沟区血清肿发生的原因以及如何降低发生率。方法选取2014年11月至2015年11月广西壮族自治区人民医院疝与腹壁外科腔镜治疗中心同一手术组行腹腔镜腹股沟疝无张力修补术的65例老年腹股沟疝患者，分为放置引流管组（31例）和未放置引流管组（34例）两组。对比腔镜下无张力修补病例中置引流管者与未置引流管者的液体引流量、术后体温变化、术后疼痛感、血清肿发生数量等情况。结果手术顺利完成，手术时间50～110 min，平均（92.4±22.5）min。术后随访1～12个月后统计治疗组未有血清肿发生，对照组发生血清肿5例。引流管内引流量≤20 ml或不超过72 h拔除引流管。放置引流管组第1个24 h引流量较多，其中1例高达248 ml，累计总量平均在186 ml。术后住院时间2～7 d。放置引流管组患者术后并未增加局部疼痛指数、发热等临床症状，差异有统计学意义（P〈0.05）。结论腔镜下老年人腹股沟疝修补术中置引流管能预防术后腹股沟疝区血清肿发生，该方法安全可行，具有一定的临床疗效。ObjectiveAnalysis the causes of inguinal hematoma after laparoscopic inguinal hernia repair at the elders, and how to reduce the rate of this incident.MethodsTo select the elderly hernia patients of 65 cases with a surgical group of Laparoscopic Surgery Center for Abdominal Wall Hernia center in the People＆#39;s Hospital of Guangxi zhuang Autonomous Region from November 2014 to November 2015, divided into drainage tube placement group （31 cases） and no drainage tube placement group （34 cases） in the two groups. To review the elderly hernia patients with a surgical group of my center nearly one year, they which was the liquid drainage, postoperative temperature, postoperative pain and seroma were observed after laparoscopic inguinal hernia repair.Results The operation was successfully completed, the operation time ranged from 50 to 110 min, with an average of （92.4±22.5） min. Postoperative follow-up of 1 ~ 12 months after the statistical no seroma occurred in the treatment group, 5 cases of seroma in the control group. Drainage tube drainage is ≤20 ml or less than 72 h to pull out the drainage tube. Pdrainage tube placement group in first 24 h has more drainage, including 1 case of up to 248 ml, accumulative total average in 186 ml. Postoperative hospital stay 2 ~ 7 d. Drainage tube placement group patients did not increase local pain index, fever and other clinical symptoms, the difference was statistically significant （P〈0.05）.Conclusion Placing drainage tube in laparoscopic inguinal hernia repair at the elders, could prevent the incident of seroma, which has certain feasibility, safety and effectiveness.
