摘　　要：目的:通过了解某市上级医院与社区卫生服务机构的基本服务情况,分析社区首诊中存在的问题,为某市社区首诊制度的进一步完善提出相关建议。方法:采用文献法和问卷调查方法收集资料,通过重点调查与抽样调查相结合,对某市3家综合性医院、7家社区卫生服务机构及周边居民进行问卷调查。结果:社区卫生服务机构与上级医院的医务人员在学历和执业年限上存在差距;社区卫生服务机构的日常接诊量低于上级医院的接诊量;社区医疗卫生资源不足是阻碍社区首诊有效落实的主要问题。结论:通过提高社区医疗服务的质量,加大社区首诊的宣传力度,加强居民健康管理等措施破解社区首诊的障碍。Objective To explore the present situation of health service in Rizhao city of Shandong province,to find out the problems existing in the implementation of preliminary diagnosis in community,which provides references for exploration and practice of graded diagnosis mode,and to offer relevant advices for its improvement.Methods Literature review and questionnaire survey were adopted.A combination of key-point investigation and sampling survey was conducted in 3general hospitals,7community health service institutions and some local residents with self-made questionnaire.Results There were great differences in education and professional capacity between medical staff in hospitals and the staff in community health service institutions.The reception amount of community health service institutions was less than that of hospitals.The most serious problem that hindered the development of preliminary diagnosis in community was the lack of medical resources.Conclusions Some helpful recommendations,such as improving the quality of community health service,stepping up the publicity efforts on the medical system and enhancing residents＇ health management,are helpful for preliminary diagnosis in community.
