摘  要:目的应用常规超声及血管回声跟踪(e—tracking,ET)技术初步探讨早衰症患儿颈部血管病变及弹性功能。方法对6例确诊早衰症儿童、21例正常儿童和14例衰老程度相当的健康老年人行常规颈部血管及ET检查,获得参数弹性模量、僵硬度、顺应性、膨大指数、脉搏波传导速度,将三组测量结果进行比较分析。结果①早衰组血管器质性改变:6例患儿中5例出现颈部血管异常(83.3%)。连续性观察颈部血管显示病变程度进行性加重。病情较重的1例出现多处粥样硬化斑块。②血管弹性的改变:弹性模量、僵硬度、脉搏波传导速度老年组均大于早衰组和正常儿童组(P〈0.05),顺应性健康老年组均小于早衰组和正常儿童组(P〈0.05),三组间的膨大指数差异无统计学意义(P〉0.05)。早衰组和正常儿童组间上述弹性参数差异无统计学意义(P〉0.05)。结论早衰症患儿常规超声显示颈部血管出现异常,早衰组弹性参数与老年组有明显差异,但与正常儿童组未表现出明显差异,提示早衰患儿的血管病变机制与自然衰老机制不同,血管弹性可能与正常儿童相近,尚需进一步研究证实。Objective To explore the carotid artery lesion and elasticity in Hutchinson-Gilford progeria syndrome (HGPS) children by ultrasound e-tracking technique. Methods Six HGPS patients, 21 age-matched children, and 14 senescence-matched healthy volunteers were examined by carotid artery ultrasound and e-tracking imaging, the parameters of pressure-strain elastic modulus (Ep), artery stiffness parameters(β), arterial compliance (AC), augmentation index (AI), and pulse wave velocity (PWVβ) were measured and analyzed. Results Organic lesion of HGPS:5 cases presented carotid artery abnormalities in the 6 HGPS patients (83.3%). Longitudinal observation showed lesions progressively aggravated with time. The most serious patient presented as multiple atherosclerosis plaques. @Carotid artery elasticity:the parameters of EP, β, PWVβ in senescence-matched healthy volunteers were significantly higher than those in the HGPS patients and age-matched children( P 〈0.05). The parameter of AC was significantly lower than that in the HGPS patients and age-matched children( P 〈0.05). There was no significantly difference in AI among the three groups ( P 〉 0.05). All the parameters showed no significant difference between HGPS patients and age-matched children ( P〈0.05). Conclusions Conventional ultrasound examination showed the carotid artery anomality in HGPS patients, whose elastic parameters by ultrasound e-tracking has obvious difference compared with senescence-matched healthy volunteers, but has no obvious difference compared with normal children, indicating that HGPS may not have the same aging process with natural senescence and carotid artery elasticity may close to normal children.
