摘　　要：目的：评价精细化管理在髋膝关节置换患者围术期排尿管理中的应用效果。方法采用便利抽样的方法,选取2015年9月—2016年3月于中日友好医院行髋膝关节置换术的患者95例,分为观察组53例,对照组42例。观察组患者按照精细化管理流程进行围术期排尿管理,对照组患者采用常规的排尿管理模式,比较两组患者术前排尿训练依从率和术后尿潴留发生率。结果观察组患者术前排尿训练的依从率84.9％,对照组40.5％；观察组患者术后尿潴留发生率22.6％,对照组47.6％；两组比较差异均有统计学意义（χ^2分别为20.403,6.544；〈0.05）。结论以精细化管理理念为指导,构建髋膝关节置换围术期的排尿管理流程,可以有效提高患者术前排尿训练的依从性,降低术后尿潴留的发生率。Objective To evaluate the effect of fine management in perioperative micturition management in patients with hip or knee replacement. Methods By convenient sampling method, 95 patients with hip or knee replacement from September 2015 to March 2016 were selected as the research object. There were 53 cases in the intervention group and 42 cases in the control group. Patients in the intervention group received perioperative micturition management according to the process of fine management, while patients in the control group received routine micturition management mode. The preoperative micturition training compliance rate and the incidence of postoperative urinary retention in two groups were analyzed and compared. Results The preoperative micturition training compliance of patients in the intervention group was 84.9%, and it was 40.5% in the control group;the incidence of postoperative urinary retention in the intervention group was 22.6%, and it was 47. 6% in the control group （χ^2 =20. 403, 6. 544;P〈0. 05 ） . Conclusions With fine management concept as the guidance, perioperative micturition management process in hip or knee replacement can effectively improve the preoperative micturition training compliance and reduce the incidence of postoperative urinary retention.
