肾康灵对维持性血液透析患者炎症状态及残余肾功能的影响

摘　　要：目的采用肾康灵治疗维持性血液透析患者,观察炎症状态和残余肾功能(RRF)的变化。方法将60例血液透析患者随机分为观察组和对照组各30例。2组均予低盐、低脂、优质蛋白饮食,控制血压,维持水、电解质和酸碱平衡,纠正贫血等,同时进行血液透析、血液透析滤过治疗。观察组在此基础上加用肾康灵,每次8片,每日3次,口服。2组均治疗6个月。观察2组治疗前后RRF、尿量及血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平。结果治疗前2组RRF、尿量无明显差异(P>0.05),治疗后2组RRF、尿量明显减少(P<0.05),观察组明显高于对照组(P<0.05)。2组治疗前血清hs-CRP、TNF-α、IL-6水平差异无统计学意义(P>0.05),治疗后观察组血清hs-CRP、TNF-α、IL-6水平均明显降低,且低于对照组(P<0.05)。结论肾康灵可以保护维持性血液透析患者的RRF,减轻炎症反应。Objective To observe the inflammatory state and residual renal function（RRF） of the patients with maintenance hemodialysis treated through Shenkangling. Methods Sixty patients with maintenance hemodialysis were randomly divided into two groups, the observation group（30 cases） and the control group（30 cases）. They were treated through hemodialysis and hemodiafiltration. The patients in the observation group were treated with Shenkangling 8 tablets/time, 3 times/day. RRF and urine volume after six months were evalued. Serum high-sensitivity C reactive protein（hs-CRP）, tumor necrosis factor alpha（TNF-α）, and leukocyte mediated IL-6（IL-6） before and after treatment were detected. Results There were no statistical differences in RRF and urine volume between the two groups before treatment（P〉0.05）. After six months, RRF and urine volume in the two groups were significantly reduced（P〈0.05）, and those of the observation group were significantly higher than those of the control group（P〈0.05）. There was no statistical significance in serum hs-CRP, TNF-α and IL-6 between the two groups before treatment（P〉0.05）. After treatment, the serum levels of hs-CRP, TNF-α, IL-6 significantly decreased in the observation group, and lower than those of the control group（P〈0.05）. Conclusion Shenkangling can protect the residual renal function of hemodialysis patients, and reduce the inflammatory reaction.
