摘 要:目的探讨CysC和KIM-1在造影剂肾病早期预测中的价值。方法选择2014年6月至2015年5月在我院行冠状动脉造影术的患者40例,设定为观察组,另取同期40例未进行冠状动脉造影术患者作为对照组,应用免疫比浊法、酶联免疫吸附法等比较两组术后4、12、24、48和72 h时的CysC、KIM-1、血尿素氮、血清肌酐及e GFR水平。结果术后4 h两组CysC比较差异无统计学意义(P>0.05),术后12、24、48和72 h时观察组CysC均高于对照组,且在24 h时达到最大值,差异有统计学意义(P<0.05);观察组术后4、12、24、48和72 h时KIM-1均高于对照组,且在12 h时达到最大值,差异有统计学意义(P<0.05)。术后4、12、24、48 h时两组血尿素氮、血清肌酐及eGFR比较差异均无统计学意义(P>0.05);观察组血尿素氮及eGFR在术后72 h高于对照组,差异有统计学意义(P<0.05);观察组血清肌酐在术后72 h与对照组比较,差异无统计学意义(P>0.05)。结论 CysC和KIM-1相较传统血尿素氮、血清肌酐及eGFR等指标,对造影剂肾病的早期预测具有更高的价值。Objective To investigate the predictive value of CysC and KIM-L in the early prediction of contrast-induced nephropathy.Methods A total of 40 cases who received coronary angiography were selected from June2014 to May 2015 as observation group,and 40 cases without coronary angiography in the same period of time were selected as control group.The levels of CysC,KIM-1,blood urea nitrogen,serum creatinine and eGFR were detected by immune turbidimetry,enzyme-linked immunosorbent assay methods in postoperative 4 h,12 h,24 h,48 h and 72 h.Results CysC in observation group in postoperative 4 h had no statistical difference compared with control group(P〉0.05),but in postoperative 12 h,24 h,48 h and 72 h it was higher than that of control group and the maximum appeared in postoperative 24 h,the difference had statistical significance(P〈0.05).KIM-1 in observation group in postoperative 4 h,12 h,24 h,48 h and 72 h was higher than that of control group and the maximum appeared in postoperative 12 h,the differences being statistically significant(P〈0.05).The blood urea nitrogen,serum creatinine and eGFR in postoperative 4 h,12 h,24 h,48 h had no statistical difference between the two groups(P〉0.05);the blood urea nitrogen and eGFR in observation group were higher than those of control group in postoperative 72 h,the differences being statistically significant(P〈0.05);the serum creatinine had no statistical difference in postoperative 72 h between the two groups(P〉0.05).Conclusion CysC and KIM-1 have high value in early prediction of contrast-induced nephropathy compared with blood urea nitrogen,serum creatinine and eGFR.
