骨松强骨方治疗肾阳虚血瘀型原发性骨质疏松症疗效观察

摘　　要：目的观察骨松强骨方治疗肾阳虚血瘀型原发性骨质疏松症的临床疗效。方法将80例肾阳虚血瘀型原发性骨质疏松症患者随机分成2组,实验组50例给予骨松强骨方治疗,对照组30例给予仙灵骨葆胶囊治疗,疗程12周。观察2组治疗前后骨痛评分、中医临床症状评分、Berg平衡量表评分、骨代谢指标及骨密度的变化情况,统计2组总体疗效、不良反应发生情况及脆性骨折发生率。结果 2组治疗后骨痛评分、中医临床症候评分、Berg平衡量表评分均明显改善(P均<0.05),且实验组各项评分改善情况均明显优于对照组(P均<0.05);实验组治疗后β-CTX明显低于治疗前及对照组(P均<0.05),T值和显效率、总有效率均明显高于对照组(P均<0.05),骨折发生率明显低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论骨松强骨方可有效改善肾阳虚血瘀型骨质疏松患者疼痛、平衡能力及中医临床症状,防止骨密度下降,抑制骨吸收,降低脆性骨折发生率,且长期服用安全,疗效更好。Objective It is to observe the clinical effects of Gusong Qianggu decoction（ GSQGD） on the primary osteoporosis of Yang deficiency of kidney and blood stasis type. Methods Eighty patients with primary osteoporosis of Yang deficiency of kidney and blood stasis type were randomly divided into two groups,the experimental group（ n = 50） treated with GSQGD and control group（ n = 30） treated with Xianling Gubao capsule. Both groups were treated for 12 weeks. The changes of bone mineral density,pain score,bone metabolism markers,Chinese medical clinical symptom scores,Berg balance amount scale scores were observed before and after treatment in both groups,and incidence rate of bone fragile fracture and side effect,total curative effect were investigated after treatment. Results After treatment,bone pain scores,Chinese medicine clinical syndrome scores,Berg balance amount scale scores were significantly improved in both groups（ P〈0. 05）,and the improvements of these scores in experimental group were better than that in control group（ P〈0. 05）. The level of β-CTX after treatment in experimental group was significantly lower than that before treatment and in control group after treatment（ P〈0. 05）,T value,marked improvement rate,total effective rate were higher while incidence rate of bone fracture was lower in experimental group than that in control group. There was no significant difference in incidence of side effect between the two groups（ P〉0. 05）.Conclusion Gusong Qianggu Decoction could effectively improve pain,balance ability and clinical symptoms of TCM,prevent the decrease of bone mineral density,inhibit bone resorption and decrease the incidence rate of bone fragile fracture in the patients with primary osteoporosis of Yang deficiency of kidney and blood stasis type,furthermore,longer administration is safe with better curative effect.
