摘 要:目的临床发现侵袭性念珠菌病早期,患者抗念珠菌Eno抗原Ig G抗体水平升高,本文拟用动物实验探讨该现象是否与念珠菌定植导致的体液免疫回忆应答有关。方法小鼠用随机数字表法分为4组:第1组小鼠先行口腔定植念珠菌而后进行腹腔感染(定植后感染免疫正常组,n=8),第2组在第1组基础上外加免疫抑制处理(定植后感染免疫抑制组,n=8),第3组给予念珠菌定植,不进行腹腔感染(定植未感染免疫正常组,n=4),第4组不给予念珠菌定植,直接进行腹腔感染(未定植感染免疫正常组,n=4)。检测免疫正常和免疫抑制状态的感染小鼠脾中念珠菌烯醇化酶(Eno)特异的记忆B细胞(Bm)的数量以及血清抗Eno Ig G、Ig M和Ig A水平。结果定植后感染免疫正常组和定植后感染免疫抑制组小鼠Eno抗原特异性Bm数(47.25±13.81、43.14±15.95)明显多于定植未感染免疫正常组和未定植感染免疫正常组(8.00±3.74、8.50±2.38),差异有统计学意义(P<0.01);而定植后感染免疫正常组和定植后感染免疫抑制组、定植未感染免疫正常组与未定植感染免疫正常组间差异无统计学意义(P>0.05)。念珠菌定植后感染的小鼠血清抗Eno Ig G水平较Ig M、Ig A水平显著升高;抗Eno Ig G水平与特异性Bm频数呈正相关(r=0.737,P<0.01)。抗Eno Ig G水平与特异性Bm频数呈正相关(r=0.737,P<0.01)。结论念珠菌定植并在遭受侵袭性感染后特异性记忆细胞短时大量增殖可致Eno特异性Ig G抗体水平快速升高。Objective Rapid elevation of the IgG antibody against Candida Enolase( Eno) has been observed in patients with invasive candidosis in an early stage. The present study was to confirm the association of Candida colonization with humoral immune anamnestic response of the dominant antigen. Methods Twenty-four mice were randomized into group 1 treated by oral Candida colonization plus intraperitoneal infection( immunocompetent,n = 8),2 treated with immunosuppressant in addition to the treatment of group 1( immunocompromised,n = 8),3 treated by oral Candida colonization only( immunocompetent,n = 4) and 4 treated by intraperitoneal injection only( immunocompetent,n = 4). The number of Eno-specific memory B-cells in the spleen and the levels of IgG,IgM and IgA antibodies were determined in the peripheral blood of the immunocompetent and immunocompromised invasive candidiasis mice.Results At 7 days after invasive infection,there were significantly more Eno-specific memory B-cells in the mice of groups 1( 47. 25 ±13.81) and 2( 43.14± 15.95) than in groups 3( 8.00± 3. 74) and 4( 8.50±2.38)( P〈0.01),with no statistically significant differences between either groups 1 and 2 or groups 3 and 4( P〉0.05). Eno-IgG antibodies were detected in the serum of the mice of the first two groups in the early stage of invasive infection and positively correlated with antigen-specific memory B-cells( r = 0.737,P 〈0.01). Conclusion Rapid elevation of the Eno-IgG antibody level in the early stage of invasive infection after Candida colonization may be attributed to the rapid proliferation of humoral immune memory cells.
