健脾止动汤对多发性抽动症儿童HPA轴功能的影响

摘　　要：目的观察健脾止动汤对多发性抽动症(TS)儿童下丘脑-垂体-肾上腺轴(HPA)功能状态的影响,探讨HPA轴在TS中的作用。方法选取符合美国《精神病诊断与统计手册》(第4版)TS诊断标准的56例患儿作为观察组,30例健康儿童作为对照组,分别检测其血清皮质醇(COR)和促肾上腺皮质激素(ACTH)水平。观察组予口服健脾止动汤配方颗粒,疗程12周,对其中数据完整的48例TS,于疗后再次检测其血清COR和ACTH水平。结果观察组患儿血清COR和ACTH水平明显高于对照组,差异具有统计学意义(P<0.001)。观察组治疗后血清COR和ACTH水平均低于治疗前,差异具有统计学意义(P<0.01)。结论 TS儿童存在着HPA轴功能亢进表现,健脾止动汤可下调HPA轴水平。Objective To observe the influence of Jian-Pi-Zhi-Dong Decoction（ JPZDD） on hypothalamic pituitary adrenal（ HPA） axis,and explore the role of HPA axis in TS. Methods Fifty-six cases of children with TS were given JPZDD for treatment（ treatment group）,and thirty healthy children were recruited as control group. The serum levels of cortisol（ COR） and adrenocorticotrophic hormone（ ACTH）in both groups were examined by enzyme-linked immunosorbent assay（ ELISA）. Furthermore,all of the hormones were tested after the 12 weeks treatment of JPZDD in the treatment group. Results Compared with the control group,the contents of COR and ACTH increased significantly in the treatment group（ P〈0. 001）. After 12 weeks of treatment,the contents of COR and ACTH in the treatment group dramatically decreased（ P〈0. 01）. Conclusion The hyperfunction of HPA plays an important role in TS,and JPZDD can significantly reduce the levels of COR and ACTH,which may contribute to the function of anti-tic.
