重症急性胰腺炎合并心肌梗死1例救治体会

摘　　要：重症急性胰腺炎（severe acute pancreatitis,SAP）是常见的急腹症之一,病情凶险,常并发急性呼吸窘迫综合征（acute respiratory distresssyndrome,ARDS）、上消化道出血、急性肾功能衰竭、心肌损伤,甚至多脏器功能障碍综合征（multiple organ dysfunction syndrome,MODS）。To investigate the pathogenesis and treatment of severe acute pancreatitis combined with myocardial infarction,and to provide a new idea for the treatment of acute pancreatitis.The clinical features and treatment measuresin one case of 66-year-old female patient with severe acute pancreatitis combined with acute myocardial infarction was reported.The experience of diagnosis and treatment was summarized.The patient was suggested to strictly stay in bed.The defecation was maintained unobstructed.The intake and output were controlled in 24 h.The biochemical indexes were monitored.The inhibition of pancreatic secretion,antiplatelet and anticoagulant therapy measures wasperformed.Finally,the patient recovered and was discharged.In addition to the traditional drug treatment,for the patient with severe acute pancreatitis combined with myocardial infarction,theurine volume,heart rate,blood pressure,central venous pressure and other indicators should be focused,the transfusion volume should be adjusted.
