维持性血液透析患者高敏心肌肌钙蛋白T水平及其与左心室结构的关系

摘　　要：目的 探讨维持性血液透析（MHD）患者高敏心肌肌钙蛋白T（hs-cTnT）的变化及其与左心室结构的关系.方法 选择48例慢性肾衰竭行规律血液透析患者为维持性血液透析组,30名健康体检者为对照组,分别测定血清hs-cTnT、血清脑钠肽（BNP）等生化指标,应用心脏超声心动图测量相关参数,计算左心室质量指数（LVMI）,分析hs-cTnT水平及其与左心室结构的关系.结果 维持性血液透析组hs-cTnT水平与对照组比较明显升高[（138.70＆#177;158.62）ng/ml比（0.22＆#177;0.64）ng/ml],差异有统计学意义（P＜0.05）.单因素相关分析显示,维持性血液透析患者hs-cTnT与LVMI、BNP、收缩压、年龄呈正相关（r=0.513,P=0.000;r=0.438,P=0.001;r=0.317,P=0.003;r=0.304,P=0.032）.Logistic多元回归分析显示,hs-cTnT与LVMI及BNP呈独立正相关.结论 维持性血液透析患者hs-cTnT水平升高.Hs-cTnT可作为左心室肥厚预测因子,有助于维持性血液透析患者心血管疾病的早期风险评估.Objective：To study the changes of serum highly sensitive cardiac troponin （hs-cTnT） in patients with maintenance hemodialysis （MHD） and its relationship with left ventricular structure.Methods： 48 patients with chronic renal failure taking maintenance hemodialysis were randomly selected as case group , 30 cases of healthy outpatients without kidney disease were as control group. Serum hs-cTnT、BNP and biochemical parameters were measured. The correlation parameters were measured by echocardiography, and the left ventricular myocardial mass index （LVMI） was calculated, and the relationship between hs-cTnT level and left ventricular structure was analyzed.Results： Hs-cTnT in MHD group was significantly higher than those in control group（P〈0.05）.Univariate analysis showed that hs-cTnT was positively associated with LVMI、BNP、systolic blood pressure and agein maintenance hemodialysis patients（r=0．513 P=0.000 r=0.438 P=0.001 r=0.317 P=0.003 r=0.304 P=0.032）. Logistic multi-regression analysis showed that the hs-cTnT was independently and positively correlated with systolic LVMI and BNP.Conclusions： Hs-cTnT levels were elevated in patients with MHD. Hs-cTnT can be used as a predictor of left ventricular hypertrophy, which is helpful for early risk assessment of cardiovascular disease in MHD patients.
