红细胞体积分布宽度与血清同型半胱氨酸对高血压早期肾损伤的临床价值研究

摘　　要：目的:探讨红细胞体积分布宽度(RDW)与血清同型半胱氨酸(s HCY)联合检测对高血压早期肾损伤的临床价值。方法:选取248例高血压病患者(高血压组)、105例健康体检者(对照组)作为研究对象,根据尿微量白蛋白(m ALB)、β2-微球蛋白(β2-MG)将高血压组患者再分为单纯高血压组(145例)、高血压早期肾病组(103例),检测所有研究对象RDW、s HCY。结果:三组研究对象RDW、s HCY差异有统计学意义(P<0.05),高血压早期肾病组RDW、s HCY显著高于单纯高血压组(P<0.05),单纯高血压组RDW、s HCY显著高于对照组,差异有统计学意义(P<0.05)。三组研究对象RDW、s HCY与尿m ALB及β2-MG呈正相关关系,RDW与s HCY随着尿m ALB及β2-MG含量的增多而增加。结论:RDW与s HCY联合检测可以提高高血压早期肾损伤的检出率。Objective To investigate the clinical value of combined detection of red blood cell volume distribution width （RDW） and serum homoeysteine （sHCY） in patients with early renal injury in hypertension. Method 248 eases of hypertension （hypertension group） and 105 healthy subjects （control group） were selected as the research objects. According to the urine microalbumin （mALB） and beta 2 - microglobulin （ β2 - MC） will be a simple hypertension group （145 cases of hypertension group were subdivided into） , early hypertensive nephropathy group （103 cases） and detection of all research objects RDW, sHCY. Results The differences of sHCY and RDW in the 3 groups were statistically significant （ P 〈 0. 05 ）, The RDW and sHCY in the early stage of hypertension were significantly higher than those in the simple hypertension group （ P 〈 0. 05 ）. The RDW and sHCY of the hypertension group were significantly higher than those in the control group（ P 〈 0. 05 ）. The positive correlation between RDW, sHCY and urine mALB and β2 - MG was positive correlation between and RDW, and sHCY increased with the increase of urinary mALB and β2 - MG content. Conclusion Red blood cell distribution width and serum homocysteine detection can improve the detection rate of high blood pressure early renal injury.
