锁骨钩板与袢钢板治疗肩锁关节脱位的临床疗效分析

摘　　要：目的：探讨锁骨钩钢板和袢钢板治疗肩锁关节脱位的临床疗效,为手术方案的制定提供指导。方法：选取48例肩锁关节脱位患者的临床资料进行回顾性分析,分别采用锁骨钩钢板（24例）和袢钢板（24例）固定,术后随访1~2年,行Constant-Murley评分及Karlsson评分评定,并观察不良反应发生情况。结果：所有患者获得随访,袢钢板组手术时间、住院费用高于钩板组;而术中出血量两组差异无统计学意义（P〉0.05）;术后3个月两组Constant-Murley和Karlsson评分优良率差异无统计学意义（P〉0.05）;术后6个月和1年袢钢板组评分优良率均高于钩板组,差异有统计学意义（P〈0.05）;袢钢板组并发症发生率明显低于钩板组。结论：袢钢板治疗肩锁关节脱位可有效恢复关节功能,减少并发症,值得临床推广应用。Objective To investigate the clavicular hook plate and loop plate fixation in the treatment of shoulder joint dislocation of the clinical curative effect of lock, provide guidance for making operation plan. Method 48 clinical data of cases of patients with shoulder division were retrospectively analyzed,respectively,using clavicular hook plate （24 cases） and loop plate （24 eases） fixed, postoperative follow up for 1 to 2 years,the constant Murley score, Karlsson score, and observe the occurrence of ad- verse reactions. Results All patients were followed up and endobutton groups in operation time, hospitalization expenses were higher than that of hook plate group, and intranperative bleeding in two groups had no significant difference （P 〉 0. 05） ;postoperative 3 months in both groups of constant Murley and Karlsson score the excellent and good rate hat no significant difference （P 〉0. 05）. After 6 months and 1 year endobutton group score, the excellent and good rate were higher than those of hook plate group （P 〈 0.05 ）; endobutton complications occurred were significantly lower than those of the hook plate , group. Conclusion Plate for treatment of acromioclavicular joint dislocation can effectively restore joint function, reduce complications, worthy of clinical application.
