摘 要:目的:探讨颅脑外伤患者的血清白蛋白水平变化与脑损伤程度的相关性,为辅助诊断急性颅脑损伤提供参考,减少迟发性脑损伤的发生率。方法:选取159例急性颅脑外伤患者,依据Glasgow评分,分为轻型组、中型组和重型组(各53例)。同时,选取在我院治疗的癫痫患者53例作为对照组。4组在入院第2、7、14天清晨空腹采静脉血,检测血清白蛋白、丙氨酸转氨酶(ALT)水平。结果:急性颅脑损伤程度越重,血清白蛋白水平下降越明显,ALT水平上升越明显。急性颅脑损伤程度与血清白蛋白水平呈负相关(r=-0.556,P=0.003),与ALT呈正相关(r=0.432,P=0.000)。不同时间点的血清白蛋白水平具有显著差异(P<0.05);与对照组比较,轻型组的血清白蛋白水平变化无统计学意义(P>0.05),中型组、重型组血清白蛋白水平变化有统计学意义(均P<0.05)。不同时间点的血清ALT水平具有显著差异(均P<0.05);时间变化和颅脑损伤之间存在交互效应(均P<0.05)。结论:血清白蛋白水平变化与颅脑损伤程度呈负相关,血清白蛋白水平变化越大,患者颅脑损伤程度越重。Objective: To investigate serum protein changes and their correlation with acute traumatic brain injury in order to diagnose traumatic brain injury and reduce the incidence of delayed cerebral injury. Methods: One hundred and fiftynine cases of traumatic brain injury were selected,and they were divided into mild group,moderate group and severe group in terms of Glasgow grading( 53 cases in each group). Also,53 cases of epilepsy in our hospital were selected as control group. The serum albumin and ALT levels were measured on the day 2,7 and 14 after admission. Results: The more severe traumatic brain injury,albumin decreased more significantly,ALT rise more obviously. Acute traumatic brain injury was negatively correlated with albumin levels( r =- 0. 556,P = 0. 003),but positively with ALT( r = 0. 432,P = 0. 000). There was significant difference among various time points in plasma albumin( P〈0. 05). Compared to control group,albumin level in mild group showed no significant difference( P〈0. 05),and there was significant difference between severe and moderate groups( all P〈0. 05). There was significant difference among various time points in plasma ALT( all P〈0. 05).Interactive effect existed between traumatic brain injury and time( all P〈0. 05). Conclusion: The serum albumin was negatively related with the degree of traumatic brain injury,and low serum albumin level indicates severe traumatic brain injury.
