摘  要:目的:探讨联合检测血清降钙素原和C反应蛋白在小儿肺炎定性诊断中的临床意义。方法:回顾性分析本院50例确诊为细菌性肺炎(28例)和病毒性肺炎(22例)患儿的临床资料,探讨患儿血清降钙素原和C反应蛋白水平与患儿肺炎病原学之间的联系。以上病例均经我院实验室病原学检查及血清学检查等临床实验确诊。结果:细菌性肺炎组的血清降钙素原、C反应蛋白水平分别为(2.87±0.043)ng/ml、(17.49±9.57)mg/L;病毒性肺炎组血清降钙素原、C反应蛋白水平分别为(0.41±0.19)ng/ml、(13.34±8.26)mg/L;细菌性肺炎组的血清降钙素原、C反应蛋白水平分别高于病毒性肺炎组,差异有统计学意义(P〈0.05)。细菌性肺炎组的PCT、CRP检测阳性率分别为92.9%、57.1%,均高于病毒性肺炎组;PCT指标诊断细菌性肺炎的特异性、灵敏度均高于CRP指标;差异有统计学意义(P〈0.05)。结论:血清降钙素原和C反应蛋白水平在小儿细菌性肺炎中均有所升高;血清降钙素原和C反应蛋白的联合检测,可判断小儿肺炎的病原学种类;PCT指标诊断细菌性肺炎较CRP指标具有更高的特异性、灵敏度。Objective: To investigate the application of combined detection of serum procalcitonin (PCT) and C-reactive protein (CRP) in the diagnosis of Pediatric pneumonia. Methods: A retrospective analysis was designed to explore the relationship of serum procalcitonin (PCT) and C-reactive protein (CRP) levels with the type of children pneumonia. All the 50 cases (bacterial infection group, n=28; virus infection group, n=22) were confirmed by pathogenic and serologic examination in our clinical laboratory. Results: The PCT, CRP levels of bacterial infection group were (2.87±0.043) ng/ml, (17.49±9.57) mg/L respectively. The PCT, CRP levels of virus infection group were (0.41±0.19) ng/m], (13.34±8.26) mg/L respectively, The levels of PCT, CRP in bacterial infection group were significantly higher than that of virus infection group and had statistically significant difference (P〈0.05) . The positive rates of PCT and CRP in the bacterial infection group were 92.9% and 71.4%. The specificity and sensitivity of PCT was higher than that of CRP in bacterial infection group. These differences all had statistical significance (P〈0.05) .Conclusion: The serum procalcitonin (PCT) and C-reactive protein (CRP) had certain values in identifying bacterial infection and virus infection. PCT had a good value for the identification of bacterial infections. Combined detection of PCT and CRP showed important significance in differential diagnosis of the type of children pneumonia.
