重组人α-2b干扰素凝胶结合液氮冷冻治疗肛周尖锐湿疣的临床观察

摘　　要：目的观察重组人α-2b干扰素凝胶结合液氮冷冻治疗肛周尖锐湿疣(CA)的临床效果。方法回顾性分析2014年2月至2015年12月在我院接受治疗的80例肛周CA患者的临床资料,并根据其治疗方式分为对照组(n=42)和观察组(n=48),对照组给予液氮冷冻治疗,观察组给予重组人α-2b干扰素凝胶结合液氮冷冻治疗。比较两组有效率,治疗前后皮损面积、炎症细胞因子水平和复发情况。结果观察组治疗的有效率为97.37%,显著高于对照组(83.33%),差异有统计学意义(P<0.05)。治疗前,两组皮损面积差异无差别(P>0.05);治疗后,两组皮损面积均分别较治疗前缩小,观察组皮损面积显著小于显照组(P<0.05)。治疗前,两组IL-18、IL-6和hs-CRP水平无差异(P>0.05);治疗后,两组上述指标均分别较治疗前降低、观察组上述指标水平分别显著低于对照组(P<0.05)。观察组半年内复发率为7.89%,显著低于对照组(26.83%),差异有统计学意义(P<0.05)。结论重组人α-2b干扰素凝胶结合液氮冷冻治疗肛周CA效果较单用液氮治疗更优,可有效降低复发率,具有一定的临床应用价值。Objective To investigate the therapeutic effect and risk of recurrence of recombinant human interferon alpha -2b in com- bination with liquid nitrogen in the treatment of anal CA. Methods Patients with condyloma acuminatum （CA） treated in our hospital from February 2014 to December 2015 were recruited as the study subjects. Patients were divided into control group and treatment group. Patients in the control group received recombinant human interferon alpha-2b, and patients in the treatment group received re- combinant human interferon alpha-2b combined with liquid nitrogen cryotherapy. The therapeutic effect, skin lesions area, inflammatory cytokine levels and the recurrence rate were compared between the two groups before and after treatment. Results The effective rate in the observation group was 97.37%, which was significantly higher than that in the control group （83.33%, P〈0.05）. There was no sig- nificant difference in the area of skin lesions between the two groups before the treatment. After the treatment, the lesion area decreased in both groups and was significantly lower in the treatment group （P〈0.05）. There were no significant differences in the levels of IL-18, IL-6 and hs-CRP between the two groups before treatment. After treatment, levels of IL-18, IL-6 and hs-CRP decreased significantly and were significantly lower in the treatment group （P〈0.05）. Three patients in the observation group had recurrence of disease. The recurrence rate （7.89%） was significantly lower than that of the control group （26.83%, P〈0.05）. Conclusion Recombinant human in- terferon alpha -2b combined with liquid nitrogen freezing has favorable therapeutic effect on anal condyloma condyloma, and has low recurrence rate. It has significant clinical potential.
