门诊不合理处方的药师点评与干预

摘　　要：目的为提高南京军区福州总医院门诊处方质量,促进临床合理用药,对不合理处方干预情况进行分析。方法对南京军区福州总医院门诊2014年9月—2015年9月期间处方干预情况进行统计分析。结果不合理总处方数共计1640张,其中不规范处方100张,占6.10%;用药不适宜处方1447张,占88.23%;超常处方93张,占5.67%。用药不适宜处方中以药品剂型或给药途径、用法用量不适宜为主,分别占不合理处方总数的25.12%和55.61%。结论药师及时的处方干预能有效地保障患者的用药安全,减少因不合理用药而产生的医疗纠纷,保证医疗安全。Objective To improve the quality of outpatient prescriptions in our hospital and to promote clinical rational drug use.Methods Cases of intervention with prescriptions at our out-patient pharmacy from September 2014 to September 2015 were statistically analyzed.Results Among the 1640 improver prescriptions which were intervened with,there were 100non-standardized prescriptions（6.10%）,1447 prescriptions of inappropriate drug use（88.23%）and 93 excessive prescriptions（5.67%）.Prescriptions of inappropriate drug use mainly involved as inappropriate dosage forms or routes of administration,and incorrect dosage or usage of drugs,accounting for 25.12% and 55.61%,respectively.Conclusion Pharmacists′timely intervention with prescriptions can effectively protect the patients′medication safety,reduce medical disputes caused by irrational drug use and ensure medical safety.
