摘 要:目的探讨经腹三维超声容积对比成像联合C平面技术(VCI-C)对胎儿小脑蚓部发育异常的诊断价值。方法以2014年1月~2016年6月产前超声检查的4200例孕妇为研究对象,产前超声检查、系统胎儿检查显示疑似小脑蚓部发育异常30例,从剩余正常胎儿中抽取50例为对照组,均接受经腹三维超声容积对比成像联合C平面技术检查,观察胎儿小脑蚓部发育正常、异常声像图表现,测量并比较正常与异常胎儿平均BT、BV。结果 30例疑似小脑蚓部发育异常胎儿经VCI-C检查显示单纯后颅窝池增宽11例,Dandy-Walker变异8例,Blake's囊肿7例,Dandy-Walker畸形4例,与MRI颅脑正中矢状面检查结果一致。Dandy-Walker变异、Blake's囊肿、Dandy-Walker畸形胎儿平均BV、BT均显著大于对照组(P<0.05),但单纯后颅窝池增宽与对照组比较无显著差异(P>0.05)。结论经腹三维超声容积对比成像联合C平面技术在胎儿小脑蚓部发育异常诊断中有重要作用。Objective: To investigate the value of transabdominal 3D ultrasound volume contrast imaging combined with C plane (VCI-C) in diagnosing abnormal development of cerebellum vermis in fetuses. Methods. A total of 4200pregnant women who underwent prenatal ultrasonography between January 2014 and June 2016 were studied. Prenatal ultrasonography and systematic fetal examination showed suspected abnormal development of cerebellum vermis in 30 cases. Fifty cases were selected from the remaining normal fetuses who all underwent transabdominal 3D ultrasound VCI-C. The sonographic manifestations of fetuses with normal and abnormal development of cerebellum vermis were observed. The average BT and BV of normal and abnormal fetuses were measured and compared. Results: VCI-C of 30 cases with suspected abnormal development of cerebellum vermis showed simple posterior cranial fossa widening in 11 cases, Dandy Walker variationin 8 cases, Blake' s cyst in 7 cases and Dandy Walker malformation in 4 cases, which was consistent with the results of MRI sagittal plane examination. The The average BT and BV of fetuses with Dandy-Walker variation, Blake' s cyst and Dandy-Walker malformationwere significantly higher than those of the control group (P〈 0.05) , but there was no significant difference in simple posterior cranial fossa widening (P〉0.05) . Conclusion: VCI-C plays an important role in the diagnosis of abnormaldevelopment of cerebellum vermis in fetuses.
