摘 要:目的探讨2型糖尿病周围神经病变的高压氧综合疗效及其神经肌电生理的影响。方法随机将96例2型糖尿病周围神经病变患者分为研究组(HBO组)48例,药物治疗组(对照组)48例,两组均在药物有效控制血糖的基础上以甲钴胺营养神经,而研究组在对照组的基础上,加用高压氧治疗法。动态观察和分析周围神经病变的肢体感觉神经及运动神经的传导速度、血生化改变及综合疗效评估有效率等,并与对照组比较。10天为一疗程,每两疗程间休息3~5天,共4疗程。所有患者均在治疗前后做肌电图检查,分别观察运动神经传导速度的改变变化,并进行对比研究;评价治疗前后症状水平改善。结果高压氧综合治疗组总有效率(87.50%)明显高于对照组(60.42%),两组之间具有统计学差异(P〈0.05)。HBO组较对照组的运动神经和感觉神经传导速度显著增快(P〈0.05),CHOL、TG、血浆黏度、全血低切还原黏度以及随机血糖值均较对照组显著改善(P〈0.05)。结论高压氧联用甲钴胺综合治疗2型糖尿病周围神经病变可有效地改善运动神经和感觉神经的传导速度,控制和改善2型糖尿病周围神经病变症状作用优于单纯药物组,起效快、效果好、安全性高。Objective: To investigate the clinical effects of hyperbaric oxygen on the treatment of patients with type 2 diabetic peripheral neuropathy( DPN) and the changes of the nerve and muscle electrical activity. Methods: 96 cases clinically selected of patients with type 2 diabetes peripheral neuropathy were randomly divided into two groups: 48 cases of hyperbaric oxygen combined treatment group( HBO group),48 cases of simple medicine treatment group( control group),Both groups received strict treatment regimen to control blood sugar,based on the use of mecobalamin neurotrophic treatment,and HBO group on the basis of the control group and hyperbaric oxygen treatment. The synthetic effect of follow-up and analysis were made in clinical data of both groups,such as peripheral neuropathy assessment efficiency and limb sensory and motor nerve conduction velocity,blood biochemical indicators of clinical data,and set up the control group for comparison. 10 d for a course of treatment,a total of 4 courses of treatment,each course of rest between 3 ~ 5d. All patients were examined before and after treatment,the changes of motor nerve conduction velocity were observed and compared the improvement of symptom scores before and after treatment. Results: the total effective rate of hyperbaric oxygen therapy group( 87. 50%) was significantly higher than that of the control group( 60. 42%)( P〈0. 05). In the HBO group motor nerve and sensory nerve conduction velocity compared with the control group increased significantly faster( P〈0. 05),cholesterol,glycerin three greases,plasma viscosity,low blood shear reduced viscosity and random blood glucose levels were lower than those in the control group improved significantly( P〈0. 05). The complications of the treatment group less( P〈0. 005). Conclusion: Comprehensive treatment of hyperbaric oxygen combined with Methylcobalamin in type 2 diabetic peripheral neuropathy can effectively improve the conduction velocity of motor nerve and sen
