摘 要:目的:比较局部应用重组人表皮生长因子(rh EGF)与重组人粒细胞巨噬细胞集落刺激因子(rh GM-CSF)对烧伤后残余创面愈合的影响。方法:选择2013年2月-2015年12月在我院进行诊治的烧伤后残余创面患者80例,采用自体对照的研究方法,每例患者选取2处烧伤后残余创面,随机分为EGF组和GM-CSF组,EGF组采用rh EGF凝胶治疗,GM-CSF组采用rh GM-CSF凝胶治疗。观察用药后的创面愈合时间和愈合率,选择8例患者分别在治疗前和治疗后切取创面边缘组织标本,病理学观察用药后创面组织中成纤维细胞数及毛细血管数。结果:与EGF组相比,GM-CSF组用药治疗7、14 d后创面愈合率明显升高(P〈0.05);EGF组的创面愈合时间为(16.23±4.15)d,明显长于GM-CSF组的(12.39±2.16)d(P〈0.05);用药治疗7、14 d后,两组的成纤维细胞数和毛细血管数均明显高于治疗前(P〈0.05);EGF组的成纤维细胞数和毛细血管数均明显低于GM-CSF组(P〈0.05)。结论:rhGM-CSF比rhEGF更能促进烧伤后残余创面愈合,缩短创面愈合时间,可能与其更能促进成纤维细胞的增殖或迁移,加快毛细血管的生成有关。Objective: To contrast of the curative effect of external use rhEGF and rhGM-CSF for residual burn wounds. Methods: 80 cases of patients with residual bum wounds who were treated in our hospital from January 2013 to December 2015 were selected. Using autologous comparison research method, selected 2 residual burn wounds in each patient, were divided into EGF group and GM-CSF group. EGF group was treated with rhEGF gel, GM-CSF group was treated with rhGM-CSF gel. The wound healing time and healing rate were observed, selected 8 patients for wound edge tissue samples before treatment and after treatment respectively, fiber cell number and capillary number were observed. Results: Compared with the EGF group, after treatment of 7, 14 d, the healing rate of GM-CSF group was increased significantly (P〈0.05); the wound healing time of EGF group was (16.23 ± 4.15) d, significantly longer than that of GM-CSF group(12.39± 2.16) d (P〈0.05); after treatment of 7, 14 d, the number of fibroblasts and capillary of the two groups were significantly higher compared with before treatment(P〈0.05); the number of fibroblasts and capillary of the EGF group were significantly lower that of GM-CSF group(P〈0.05). Conclusions: Compared with rhEGF, rhGM-CSF can promote residual bum wounds healing, shorten the wound healing time, and promote fibroblasts proliferation or migration, speed up the formation of blood capillary.
