显微支撑喉镜下二氧化碳激光与高频电刀切除会厌囊肿的临床疗效比较

摘　　要：目的:比较显微支撑喉镜下二氧化碳激光与高频电刀切除会厌囊肿的临床疗效。方法:回顾性分析我科自2015年1月-2015年11月收治的52例会厌囊肿患者的病例资料,按治疗方法的不同分为观察组27例,对照组25例,观察组使用显微支撑喉镜下二氧化碳激光手术,对照组支撑喉镜下高频电刀手术,比较两者手术时间、手术出血量、术后出血率、手术切除率及术后复发率之间的差异,评价两种手术方式的临床疗效。结果:观察组患者平均手术时间10.79±3.37 min,平均手术出血量3.96±1.52mL,术后无出血(0/27),手术切除率100%(27/27),术后无复发(0/27)。对照组患者平均手术时间13.57±4.11 min,手术出血量13.26±3.95 mL,术后2例出血(2/25),手术切除率84%(21/25),术后复发率20%(5/25)。两组术后出血发生率比较差异无统计学异议(p>0.05),观察组平均手术时间明显短于对照组,手术切除率明显高于对照组,手术出血量、术后复发率均显著低于对照组,差异均具有统计学意义(P<0.05)。结论:显微支撑喉镜下二氧化碳激光会厌囊肿切除术较高频电刀切除术具有手术时间短,术中出血量少,手术切除率高及复发率低的优点。Objective： To compare the clinical efficacy between CO2 laser ablation epiglottis cyst under microscopic and retaining laryngoscope and high frequency electric knife. Methods： The clinical data of 52 cases with epiglottis cyst patients treated in our department from January 2015 to November 2015 were analyzed respectively. 52 cases were divided into observe group and control group according to the different treatment methods, with 27 in observing group （OG） and 25 cases in control one （CG）. Cases in OG were undergone an operation by CO2 laser under microscopic and retaining laryngoscope, and those in CG undergone an operation by high frequency electric knife to remove the cysts. Then, an analysis was made to their clinical efficacy in a comparative way, such as the mean of operating time and bleeding volume during the operating, the bleeding after operation, the resection rate and recurrence rate. Results： In the OG group the mean of the operating time was 10.79± 3.37 min, the average of bleeding volume was 3.96± 1.52 mL. There is no post-operation bleeding and no epiglottis cyst recurrence, the resection rate is 100%. While in the CG group the mean of the operating time was 13.57± 4.11 rain min, the average of bleeding volume was 13.26± 3.95 mL. Two cases has post-operation bleeding, the resection rate was 84%, and the recurrence rate was 20%. The bleeding rate after the operation has no statistically significant （P〉0.5） between the two groups, the mean of operating time and bleeding volume ,the resection rate, the reccurence rate has statistically significant significant differe （P〈0.5）. Conclusion： Comparing to high frequency electric knife to remove the cysts, CO2 laser ablation epiglottis cyst under microscopic and retaining laryngoscope has less operating time and bleeding volume, higher resection rate and lower recurrence are smaller.
