ω-3鱼油脂肪乳在慢性阻塞性肺疾病合并自发性气胸患者术后的临床应用

摘　　要：目的观察ω-3鱼油脂肪乳在慢性阻塞性肺疾病（COPD）合并自发性气胸患者围手术期的临床疗效。方法COPD合并自发性气胸微创手术患者98例，将98例患者随机分为ω-3鱼油脂肪乳治疗组和对照组，每组患者术后均观察2周，比较两组患者术后体质指数（BMI），血气分析，血清白蛋白，总蛋白及血红蛋白量的变化，观察肺漏气的发生率及持续时间。结果行ω-3鱼油脂肪乳治疗组BMI，血气分析，血清白蛋白，总蛋白及血红蛋白量均明显增加，且高于对照组，对照组上述观察值虽较2周前有所增加，但差异无统计学意义。治疗组患者术后漏气时间及术后住院时间均较对照组显著缩短。结论COPD合并自发性气胸患者术后应用ω-3鱼油脂肪乳有助于快速康复。Objective To investigate clinical effect of ω-3 fish oil fat emulsion injection in COPD patients accompanied with spontaneous pneumothorax. Methods 98 COPD cases who complicated with spontaneous pneumothorax received minimally invasive surgery. All patients were randomly assigned to ω-3 fish oil fat emulsion treatment group and control group,. The patients in treatment group received 100 ml of ω-3 fish oil fat emulsion injection（I. V. ,q. d） ,control group were not given ω-3 fish oil fat emulsion while other therapies were the same as treatment group. Changes of body mass index（ BMI）, blood gas analysis, blood serum albumin, total protein and hemoglobin level were examined and compared immediately at 2 weeks later after operation. Results Two groups presented with improvement in BMI, albumin ,total protein and hemoglobin level., However, there were no significant difference of those indexed between before and after surgery in control group（ P 〉 0.05 ）. Compared to control group, ω-3 fish oil fat emulsion injection treated group presented with significantly higher level of albumin and hemoglobin（ P 〈 0. 05）. Conclusion Postoperative intravenous medication of ω-3 fish oil fat emulsion injection can con- tribute to recovery of COPD patients with spontaneous pneumothorax.
