医用臭氧联合类固醇激素治疗慢性腰椎间盘源性疼痛临床分析

摘　　要：目的分析腰椎旁注射臭氧联合类固醇激素治疗慢性腰椎间盘源性疼痛的临床疗效。方法慢性腰椎间盘源性疼痛患者120例，随机分为A组和B组。A组60例，注射类固醇激素混合液10ml联合纯氧20ml。B组60例，注射类固醇激素混合液10ml联合注射30％浓度的医用臭氧20ml。两组患者均给予腰椎椎旁阻滞，每隔1周注射1次，共治疗3次。观察两组患者治疗前，治疗后1周（短期）、3个月（中期）以及6个月（长期）疼痛VAS评分及Macnab疗效评价，评估各时间点两组患者的临床疗效。结果两组VAS评分在治疗后1周、3个月、6个月与治疗前比较明显降低，两组治疗前后比较差异有统计学意义（P〈0．05）；两组治疗后1周Macnab疗效评价比较差异无统计学意义（P〉0．05），治疗后3个月及6个月时比较，差异有统计学意义（P〈0．05），且B组优于A组。结论腰椎旁注射臭氧联合类固醇激素治疗慢性腰椎间盘源性疼痛临床疗效显著，可作为慢性腰椎盘源性疼痛非手术治疗的方法。Objective To assess the clinical efficacy of oxygen-ozone paravertebral injection combined with steroid in patients with chronic diseogenic low back pain. Methods A total of 120 patients with chronic diseogenic low back pain were randomly divided into two groups. Sixty patients（ group A）received a paravertebral injection of steroid mixture 10 ml with pure oxygen 20 ml. The other 60 patients （group B）received a paravertebral injection mixture 10 ml combined with oxygen-ozone 20 ml. Injections were given every a week, amount to 3 times. Clinical outcomes were assessed at l week, 3 months and 6 months after treatment by using VAS and Macnab method. Results The VAS scores of both groups were significantly decreased at follow-up（ 1 week,3 months and 6 months ） after treatment（ P 〈 0.05 ）. No significant difference was shown between two groups＇ Macnab score at 1 week after treatment （ P 〉 0.05 ） , group B＇ s Macnab scores were better than group A with significantly difference at 3 months and 6-month follow-up（ P 〈 0. 05 ）. Conclusion Paravertehral injection of oxygen-ozone combined with steroid has a better effect on relieving chronic diseogenic low back pain and keeping pain relief, which can be administrated as a first non-surgery treatment to replace steroid therapy.
