摘 要:目的:研究动态心电图在评价2型糖尿病患者心脏自主神经功能中的价值。方法:选择2012年2月~2015年2月在我院进行诊治的2型糖尿病患者260例,按照心血管自主神经功能分为损伤组152例和正常组108例,采用动态心电图检查两组的心率变异、心脏变时功能不全和心率震荡。结果:两组间平均心率相比无明显差异(P〉0.05),损伤组的最慢心率明显高于正常组(P〈0.05),最快心率和差值明显低于正常组(P〈0.05);两组间震荡初始无明显差异(P〉0.05),损伤组的震荡斜率明显低于正常组(P〈0.05);损伤组的窦性心搏间期均值的标准差、相邻R-R间期差值的均方根值和5 min心搏间期均值的标准差均明显低于正常组(P〈0.05)。结论:应用动态心电图检测心脏变时性功能不全指标和心率震荡、心率变异指标能准确评价2型糖尿病患者的心脏自主神经功能,具有较高的诊断价值。Objective: To study the value of dynamic electrocardiogram in the assessment of cardiac autonomic nervous function in patients with type 2 diabetes. Methods: 950 cases of patients with type 2 diabetes who were treated in our hospital from January 2012 to January 2015were divided into injury group (152 cases) and normal group (108 cases) according to the function of cardiovascular autonomic nerve. The heart rate variation, cardiac dysfunction and heart rate turbulence of the two groups were detected by dynamic ECG examination. Results: There was no obvious difference in average heart rate between two groups (P〉0.05), and the slow heart rate of injury group was obviously higher than that of normal group (P〈0.05), and the fastest rate and difference was significantly lower than that of normal group(P〈0.05).The turbulence onset had no obvious difference between two groups (P〉0.05), and the turbulence slope of injury group was obviously lower than that of normal group (P〈0.05); and the RR intervals mean standard deviation, phase difference between adjacent R-R root mean square, 5 min intervals mean standard deviation of injury group were significantly lower than that of normal group (P〈0.05). Conclusions: The dynamic electrocardiogram detection in the heart chronotropic function, the heart rate turbulence, the heart rate variation index, can accurately evaluate the cardiac autonomic nerve function and has high diagnostic value in patients with type 2 diabetes.
