摘  要:目的:研究肾损伤分子1(KIM-1)在扩张型心肌病(DCM)心力衰竭患者中肾功能的评估作用。方法:入选DCM心力衰竭患者68例以及同一年龄段的健康对照组68例,行超声心动图检查,检测血清肌酐、pro-BNP、尿中KIM-1的浓度,研究两组的pro-BNP、左心室射血分数(LVEF)、肌酐清除率(Ccr)、尿KIM-1变化情况。结果:与对照组相比,心衰组LVEF、Ccr明显降低,pro-BNP、KIM-1明显升高(P<0.05);心衰组中,随着心衰症状加重LVEF明显降低,pro-BNP、KIM-1明显升高(P<0.05),Ccr虽有降低,但差异无统计学意义(P>0.05);心衰组KIM-1与pro-BNP、LVEF、Ccr均明显相关(r=0.831、-0.728、-0.677,P<0.01)。结论:KIM-1能较早地反映DCM心力衰竭患者的肾功能损伤,对心力衰竭患者的病情评估、预后判断有较大价值。Objective: To evaluate the value of kidney injury molecule-1( KIM-1) on renal function with dilated cardiomyopathy heart failure( DHF). Methods: 68 patients with DHF were enrolled into the DHF group,68 agematched healthy people as control group. Creatinine clearance rate( Ccr),pro-BNP,urinary KIM-1 and left ventricular ejection fraction( LVEF) were detected and analyzed between DHF group and control group. Results:Ccr and LVEF were lower in the DHF group than those in the control group,but pro-BNP and KIM-1 were higher in the DHF group( P〈0. 05). Along with the aggravation of heart failure symptoms,LVEF gradually decreased,proBNP and KIM-1 increased significantly( P〈0. 05),Ccr didn't decrease significantly( P〉0. 05),even though Ccr decreased gradually. There was correlation betwen KIM-1 and pro-BNP,LVEF,Ccr in DHF group( r = 0. 831,- 0. 728,- 0. 677,P〈0. 01). Conclusion: KIM-1 reflects the early renal function injury in patients with haert failure,and has great value in assessing the condition and prognosis of patients with heart faiure.
