喉癌全切手术后气管感染对肺功能影响的临床观察

摘　　要：目的探讨喉癌全切手术后气管感染对患者肺功能的影响。方法 2008年8月-2012年8月期间我院收治的实施喉癌全切手术患者173例,均经病理检查证实为鳞状细胞癌。观察喉癌全切术术后气管感染发生情况;及感染患者肺功能的变化情况;并观察气管感染患者与非气管感染患者术后随访3年肿瘤复发情况。结果 173例发生气管感染20例,感染率为11.56%;术后气管感染患者FEV_1、V50明显增加,而V25明显下降,且均具有统计学差异(P<0.05);而手术前后气管感染患者FVC比较无统计学差异(P>0.05)。结论喉癌全切手术后气管感染对患者肺功能具有一定影响,临床需采取相关措施降低喉癌全切手术后气管感染发生率。Objective To investigate the effect of tracheal infection on lung function of patients with larynge- al carcinoma after total surgical resection. Methods A total of 173 patients with laryngeal carcinoma were treated in our hospital from August 2008 to August 2009, and all of them were confirmed as squamous cell carcinoma by patho- logical investigation. The incidence of postoperative tracheal infection in patients with laryngeal squamous cell carcinoma and the changes of pulmonary function were observed before and after total resection of laryngeal cancer. The recurrence of tumor after 3 year follow-up was observed too. Results There were 20 trachea infection cases, and infection rate was 11.56%. FEV1 and V50 increased significantly in patients with postoperative tracheal infection, while V25 decreased significantly （P 〈 0. 05）. There was no significant difference in FVC between the patients with tracheal infection before and after operation （P 〉 0.05 ）. Conclusion The trachea infection after total resection of laryngeal carcinoma has certain influence on their pulmonary function, and the related measures should be taken to reduce the incidence of postoperative infection.
