摘  要:目的:观察血清白介素-18(IL-18)、单核细胞趋化蛋白-1(MCP-1)和高敏C反应蛋白(hs-CRP)在慢性阻塞性肺疾病(COPD)检测中的意义。方法:选取2012年1月-2014年12月在笔者所在医院就诊的肺功能Ⅱ级以上的COPD急性加重期患者75例,为急性加重期组,根据肺功能将其分为三组,Ⅱ级组22例,Ⅲ级组26例和Ⅳ级组27例。选择同期在笔者所在医院就诊的COPD缓解期的患者30例和健康体检者20例分别为缓解期组和对照组。观察COPD急性加重期组、缓解期组和对照组的血清的IL-18、MCP-1和hs-CRP水平变化,以及急性加重期血清IL-18、MCP-1和hs-CRP水平与疾病严重程度和指标之间的相关性。结果:急性加重期组和缓解期组的IL-18、MCP-1和hs-CRP水平明显高于对照组(P<0.01),急性加重期组水平明显高于缓解期组患者(P<0.01)。COPD急性加重期患者随着分级的升高,其血清IL-18、MCP-1和hs-CRP水平也出现明显升高(P<0.01)。COPD急性加重期患者血清IL-18、MCP-1和hs-CRP水平呈正相关(r=0.345、0.431,P<0.05),IL-18和MCP-1水平呈正相关(r=0.684,P<0.05)。结论:IL-18、MCP-1和hs-CRP参与了COPD疾病的发生发展过程,其水平的高低可以用于了解病情和指导治疗,具有重要意义。Objective: To observe the significance of serum interleukin-18(IL- 18), monocyte chemoattractant protein- 1 (MCP-1) and high-sensitive C-reactive protein(hs-CRP) in patients with chronic obstructive pulmonary disease(COPD).Method: From January 2012 to December 2014, 75 patients with acute COPD whose lung function were over levelⅡ, were selected as acute exacerbation group, and were divided into 22 cases in level Ⅱ, 26 cases in level Ⅲ and 27 cases in level Ⅳ.30 patients with COPD remission at the same period and 20 healthy person were also selected as the remission group and the control grc,up. The serum levels of IL-18, MCP-1 and hs-CRP were detected in the acute exacerbation group, the remission group and the control group were observed and their correlation analysis were performed among their levels.Result: The levels of IL-18, MCP-1 and hs-CRP in the acute exacerbation group, the remission group were significantly higher than those in the control group(P〈0.01), and those levels in the acute exacerbation were significantly higher than those in the remission group(P〈0.01).The serum IL-18, MCP-1 and hs-CRP levels significantly increased in the acute exacerbation as the acute classification increased(P〈0.01).The serum IL-18, MCP-1 in the acute exacerbation were positively correlated to hs-CRP(r=0.345, 0.431, P〈0.05), there were positively correlated between IL-18 and MCP-1(r=0.684, P〈0.05).Conchlsion: The levels of IL-18, MCP-1 and hs-CRP involve in the development process of COPD, they can be used to understand the disease and guide treatment.
