跨横窦骨瓣开颅治疗骑跨横窦硬膜外血肿的效果

摘　　要：目的:研究骑跨横窦硬脑膜外血肿予以跨横窦骨瓣开颅手术治疗的效果。方法:将31例手术治疗的骑跨横窦的硬膜外血肿分成两组,15例患者行跨横窦骨瓣开颅手术为跨横窦骨瓣开颅组,16例患者行幕上骨窗联合幕下骨窗开颅手术为传统开颅组;记录两组的手术时间、术中出血量、住院天数,术前、术后24 h GCS评分,3个月后GOS评分。结果:跨横窦骨瓣开颅组手术时间明显短于传统开颅组,术中出血量及住院天数明显少于传统开颅组,两组间比较差异均有统计学意义(P<0.05);两组术后24 h GCS评分均高于术前,组内比较差异有统计学意义(P<0.05);跨横窦骨瓣开颅组术后3个月时GOS评分稍高于传统开颅组,但两组间的差异无统计学意义(P>0.05)。结论:跨横窦骨瓣开颅手术治疗骑跨横窦硬脑膜外血肿安全有效,手术时间短,术中出血少,住院天数短,能保持后颅凹的完整性。Objective： To study the effect of the epidural hematoma bestriding transverse sinus with bone flap craniotomy bestriding transverse sinus. Method： Thirty-one cases with epidural hematoma bestriding transverse sinus were divided into two groups, Fifteen patients of bone flap craniotomy bestriding transverse sinus group were acted on bone flap craniotomy bestriding transverse sinus.Sixteen cases of tradition craniotomy group were treated with supratentorial plus infratentorial bone flap craniotomy.The operation time, intraoperative bleeding, inpatient days, preoperative GCS score, 24 hours postoperative GCS score, GOS score of three months following surgery were measured.Result： Bone flap craniotomy bestriding transverse sinus group were obviously less than tradition craniotomy group on operation time, intraoperative bleeding and hospitalization days, with statistical difference（P〈0.05）.The GCS score was significant difference in both groups between preoperation and 24 hours postoperation（P〈0.05）; however, there was no significant between two groups in the GOS score of three months following surgery（P〉0.05）.Conclusion： The results shows that the bone flap craniotomy bestriding transverse sinus is safe and effective, the bone flap eraniotomy can shorten operative time, reduce intraoperative bleeding, shorter hospitalization days and maintain the integrity of the skull concave.
