沙美特罗替卡松与氨茶碱联合治疗对慢性阻塞性肺病急性加重期疗效和氧化应激的影响

摘　　要：目的探讨沙美特罗替卡松联合氨茶碱治疗慢性阻塞性肺病急性加重期(AECOPD)疗效及对患者肺功能和氧化应激的影响。方法选取2014年1月-2015年9月于重庆三峡中心医院接受治疗的200例AECOPD患者作为研究对象,按照随机数字表法随机分为对照组(n=100)和观察组(n=100)。所有患者均给予常规治疗,在此基础上,对照组给予氨茶碱治疗,观察组在对照组的基础上联合应用沙美特罗替卡松治疗。疗程结束后,比较两组患者临床疗效、肺功能、氧化应激水平及不良反应情况。结果疗效评价后,观察组治疗总有效率高于对照组(95.0%vs 86.0%,P<0.05)。治疗后,与对照组比较,观察组患者肺功能指标FEV1、FEV1/FVC升高,氧化应激指标MDA水平下降,SOD、GSH-Px水平升高,组间比较差异有统计学意义(P<0.05)。治疗期间,对照组和观察组患者均未出现严重不良反应。结论沙美特罗替卡松联合氨茶碱治疗AECOPD疗效良好,能够明显提高临床疗效,改善肺功能,减轻氧化应激,且不良反应轻微,临床上值得应用。Objective To investigate the clinical effect of Salmeterol fluticasone propionate in combination with Aminophylfine in the treatment of acute exacerbation of chronic obstructive pulmonary disease （AECOPD） and its effects on pulmonary function and oxidative stress. Methods Totally 200 patients of AECOPD who received therapy in our hospital from January 2014 to September 2015 were selected as research objects. According to random number table, the patients were divided into two groups with equal number. All patients were given conventional treatments. The control group was given Aminophylline, while the observation group was given Salmeterol fluticasone propionate in combination with Aminophylline. Then, the curative efficacy, puhnonary function, level of oxidative stress and adverse reactions were compared. Results The observation group had a therapeutic efficiency rate of 95.0%, which was significantly higher than that of 86.0% in the control group （P 〈 0.05）. After treatment, FEV1 and FEV1/FVC （reflecting pulmonary function） in the observation group were statistically higher than those in the control group （P 〈 0.05）. As to oxidative stress, the levels of SOD and GSH-Px were statistically higher and the MDA level was statistica]iy lower in the observation group in comparison with the control group （P 〈 0.05）. During the treatment, there was no severe adverse reaction in either group. Conclusions Salmetcrol fluticasone propionate in combination with Aminophylline is effective for treatment of AECOPD, which can significantly increase curative efficacy, improve pulmonary function and alleviate oxidative stress with mild adverse reactions.
