培门冬酶联合放化疗治疗结外NK/T细胞淋巴瘤疗效观察

摘　　要：目的:观察培门冬酶联合化疗治疗结外NK/T细胞淋巴瘤的临床疗效及安全性。方法:结外鼻型NK/T细胞淋巴瘤患者32例随机分为观察组17例和对照组15例。观察组采用培门冬酶、吉西他滨、奥沙利铂及地塞米松结合放疗方案治疗;对照组采用左旋门冬酰胺酶、吉西他滨、奥沙利铂及地塞米松结合放疗方案治疗。完成6个治疗周期后,比较两种治疗方案的近期疗效及不良反应。结果:27例(84.38%)患者完成全部治疗。观察组总有效率为88.24%,对照组总有效率为80.00%,差异无统计学意义(P>0.05)。对照组胃肠道反应、过敏反应的发生率高于观察组(P<0.05)。结论:培门冬酶治疗结外鼻型NK/T细胞淋巴瘤的近期疗效确切,与左旋门冬酰胺酶联合放化疗治疗比较,不良反应发生率低。远期疗效及安全性有待大样本临床观察。Objective： To observe the respective clinical effect and safety of pegaspargase combination therapy and L-asparaginase combination therapy in treating extranodal NK / T-cell lymphoma（ NKTCL）. Methods： Thirty-two patients with extranodal NKTCL,nasal type,were randomly divided into observation group and control group. Patients in observation group（ n = 17） were given pegaspargase combined with gemcitabine,oxaliplatin,dexamethasone and radiotherapy. Patients in control group（ n = 15） were given L-aspareginase combined with gemcitabine,oxaliplatin,dexamethasone and radiotherapy. Comparison of recent effect and adverse drug reactions were made between the two groups after 6 weeks treatment finished. Results： Twenty-seven patients（ 84. 38%） finished all the treatment,total effective rate of the observation group was 88. 24%（ 15 /17）,and the control group was 80. 00%（ 12 /15）. The difference between the observation and control group has no statistical significance（ P〈0. 05）. The incidence of gastrointestinal reaction and allergic reaction of the control group was higher than that of the observation group（ P〈0. 05）. Conclusion： Pegaspargase combined chemotherapy and radiotherapy had definite recent therapeutic effect in patients with extranodal NKTCL,nasal type. Compared to Laspareginase combination therapy,it had a lower incidence of adverse reactions. However,pegaspargase＇s long-term effect and safety still need further research observation with an enlargement of sample size.
