不同针灸方法治疗棘上、棘间韧带损伤疗效观察

摘　　要：目的:观察比较齐刺、温针、刺络拔罐和火针疗法治疗棘上、棘间韧带损伤的临床疗效。方法:将120例患者随机分为齐刺组、温针组、刺络拔罐组和火针组各30例,分别采用齐刺、温针、刺络拔罐及火针疗法治疗。每2~3天治疗1次,治疗5次为1疗程。比较4组患者治疗前后疼痛视觉模拟评分(VAS),并评定4组疗效。结果:温针组脱落1例,余3组均全部完成观察。4组患者治疗后VAS较治疗前降低,差异均有统计学意义(P<0.01),提示4种疗法均可以改善患者疼痛程度;4组患者治疗后VAS比较,差异有统计学意义(P<0.01)。治疗后,齐刺组VAS与温针组比较,差异无统计学意义(P>0.05);刺络拔罐组VAS分别与齐刺组、温针组比较,差异均有统计学意义(P<0.01);火针组VAS分别与齐刺组、温针组、刺络拔罐组比较,差异均有统计学意义(P<0.01)。提示火针组疗效优于齐刺组、温针组和刺络拔罐组,刺络拔罐组疗效优于针刺组及温针组。火针组愈显率、总有效率均优于齐刺组、温针组和刺络拔罐组,差异均有统计学意义(P<0.01,P<0.05);刺络拔罐组总有效率优于齐刺组、温针组(P<0.01),愈显率与2组比较,差异均无统计学意义(P>0.05);齐刺组总有效率优于温针组(P<0.05)。结论:火针治疗棘上、棘间韧带损伤疗效优于齐刺、温针、刺络拔罐疗法,刺络拔罐疗法疗效优于齐刺和温针疗法。Objective：To observe the differences among clinical effect of triple needling therapy,warm needling therapy,fire needling therapy as well as blood-letting puncture and cupping therapy in treating the injury of supra interspinous ligament.Methods：Divided 120 cases of patients into triple needling group,warm needling group,fire needling group and blood-letting puncture and cupping group randomly,30 cases in each group.Each group was respectively given triple needling therapy,warm needling therapy,fire needling therapy and blood-letting puncture and cupping therapy every two or three days,five sessions constituting one course.Compared visual analogue score（VAS） of pain scores among four groups before and after treatment and evaluated their curative effect.Results：One case of exfoliation was in warm needle group,while observations in other three groups were all finished.VAS of patients in all groups after treatment were lower than those before treatment（P〈0.01）,which implies that four types of therapies can all improve degree of pain.Comparisons of VAS of patients in four groups after treatment displayed significance in differences（P〈0.05）.After treatment,compared VAS between triple needling group and warm needling group,there was no significance in the difference（P〉0.05）.VAS in blood-letting puncture and cupping group was respectively compared with triple needling group and with warm needling group,showed significance in differences（P〈0.01）.VAS scores in fire needling group was respectively compared with triple needling group,with warm needling group and with blood-letting puncture and cupping group,differences were significant（P〈0.01）.The results indicates that curative effect of fire needling group is better than that of triple needling group,warm needling group,blood-letting puncture and cupping group,and curative effect of blood-letting puncture and cupping group is better than that of acupuncture group and warm needling group.Cure-remarkable-effectiveness rate and the tot
