环磷酰胺联合泼尼松治疗特发性间质性肺炎75例疗效观察

摘　　要：目的观察环磷酰胺联合泼尼松治疗特发性间质性肺炎(IIP)的临床疗效。方法将IIP患者150例随机分为观察组与对照组各75例,对照组给予泼尼松治疗,观察组在对照组基础上加用环磷酰胺治疗,比较2组临床疗效、呼气峰流速(PEF)、每分钟最大通气量(MVV)、一氧化碳弥散量(DLCO)、肺活量(VC)等肺功能指标及不良发应发生率。结果观察组总有效率为77.3%高于对照组的46.7%,治疗后肺功能指标PEF、MVV、DLCO、VC均明显高于对照组,不良反应发生率为14.6%低于对照组的28.0%,差异均有统计学意义(P<0.05)。结论环磷酰胺联合泼尼龙治疗IIP疗效显著,能显著改善患者呼吸功能,且不良发应较少,值得临床推广应用。Objective To observe the effects of cyclophosphamide combined with prednisone in the treatment of idiopathic interstitial pneumonia（ IIP）. Methods 150 patients with IIP were randomly divided into observation group and control group,each of 75 cases,control group were given prednisone treatment,based on the treatment of control group,observation group were treated with cyclophosphamide,the peak expiratory flow（ PEF）,maximum ventilation volume（ MVV）,diffusion capacity for carbon monoxide（ DLCO）,vital capacity（ VC） and other pulmonary function indicators,clinical efficacy and incidence of adverse effects were compared between two groups. Results The total effective rate of observation group（ 77. 3%）was better than that of control group（ 46. 7%）,after treatment lung function index PEF,MVV,DLCO,VC of observation group were better than control group,the incidence of adverse reactions in observation group（ 14. 6%） was lower than control group（ 28. 0%）,the differences were statistically significant（ P 〈0. 05） .Conclusion Cyclophosphamide combined prednisolone in treatment of IIP curative effect is remarkable,can significantly improve patients with respiratory function,and less adverse reaction,worthy of clinical popularization and application.
