宁养服务在新疆晚期癌症患者中的应用效果

摘　　要：目的探讨接受居家宁养服务与接受住院姑息治疗的晚期癌症患者生活质量的差异及宁养服务的意义。方法选取2016年1-7月新疆某三甲肿瘤医院宁养院和姑息科收治的148例各类晚期癌症患者为研究对象,根据是否接受宁养服务分为居家宁养组(72例)和姑息治疗组(76例),为居家宁养组贫困晚期癌症患者及家属提供居家免费镇痛治疗、心理与社会支持的临终关怀;为姑息治疗组贫困晚期癌症患者提供普通住院模式的对症处理。采用肿瘤患者生活质量评估量表对两组患者接受干预服务后的生活质量进行评价。结果居家宁养组患者接受干预服务前生活质量评分为(36.34±4.59)分,接受干预后为(43.03±1.68)分,差异有统计学意义(P<0.05)。姑息治疗组患者接受干预服务前生活质量评分为(35.21±5.75)分,接受干预后为(40.21±4.33)分,差异有统计学意义(P<0.05)。居家宁养组患者接受干预服务后在家庭的理解与配合、自身对癌症的认识、对治疗的态度3个方面优于姑息治疗组,差异有统计学意义(P<0.05)。结论居家宁养服务为晚期癌症患者提供了一种有效、方便、经济的医疗服务模式,是晚期癌症患者较好的选择。Objective To discuss the difference of the quality of life of patients with advanced cancer who receive hospice care at home and palliative treatment in hospital, and the significance of hospice service. Methods 148 cases of advanced cancer were selected from January 2016 to July in Xinjiang tumor hospital,who were divided into hospice care at home group （72 cases） and palliative treatment group （76 cases）. Hospice care at home group were provided free analgesia, psychological and social support for the impoverished terminal cancer patients and their families; palliative treatment groups were provided symp-tomatic treatment in hospital for poor patients with advanced cancer. The quality of life of both groups was assessed by the quality of life scale for cancer patients. Results The score of the quality of life （QOL） in t he hospice patient s group before and after the intervention were （ 36.34 ±4.59）, （43.03±1.68）, respec-tively, and the difference was statistically significant （p〈0.05）. The score of QOL in palliative treatment group before and after the intervention were （35.21±5.75）, （40.21±4.33）, respectively, and the difference was statistically significant （p〈0.05）. The score on three aspects such as the understanding and cooperation of the family, the understanding of cancer, attitude towards the treatment were better than that of the palliative care group, and the difference was statistically significant （p〈0.05）. Conclusion Hospice service provides an effective, convenient and economical medical service mode for patients with advanced cancer, and it is a better choice for patients with advanced cancer.
