摘　　要：目的:统计分析我院住院不合理用药医嘱,为临床合理用药提供参考。方法:对我院2015年1-12月登记的不合理医嘱进行统计分析。结果:共有1 633例不合理医嘱。不合理医嘱主要涉及药物剂量不当(37.84%)、出院带药无用法用量(16.66%)、给药方式错误(10.90%)、药物浓度不当(8.14%)、输液速度不当或无(8.08%)等。结论:我院住院医嘱存在一定的不合理现象,药师通过前瞻性实时审方并进行干预,可防止差错事件发生,保障临床用药安全。Objective： To analyze statistically the irrational medical advices in our hospital,and provide reference for clinical rational drug use. Methods： The irrational medical advices in our hospital were summarized and analyzed statistically from January to December in 2015. Results： A total of 1,633 irrational medical advices were reviewed. Irrational medical advices mainly contain improper drug dose（ 37. 84%）,discharge medication without usage and dosage（ 16. 66%）,wrong administration approach（ 10. 90%）,wrong drug concentration（ 8. 14%）,improper infusion speed（ 8. 08%）,etc. Conclusion： Some advices are unreasonable in our hospital,the pharmacist intervention by auditing medical orders in real time can prevent medical errors,ensure the drug safety of patients.
