法舒地尔联合舒利迭治疗慢性阻塞性肺疾病合并肺动脉高压对患者生活质量的影响

摘　　要：目的：探讨盐酸法舒地尔联合沙美特罗替卡松粉吸剂（舒利迭）对慢性阻塞性肺疾病（COPD）合并肺动脉高压（PAH）患者生存质量及预后的影响。方法：选取我院2013年1月～2014年10月入住呼吸科符合COPD及PAH诊断标准的患者。对照组给予常规抗炎解痉平喘化痰等治疗措施，法舒地尔组在对照组基础上给予法舒地尔静滴；联合治疗组在法舒地尔组基础上联合舒利迭治疗，比较三组患者治疗前、后肺功能变化、生活质量评分、呼吸困难评分及体重指数等方面的差异。结果：治疗后法舒地尔组、联合治疗组均较对照组延缓肺功能下降，降低生活质量评分及呼吸困难评分，差异均有统计学意义（P〈0．05）；联合治疗组与法舒地尔组相比，更能有效延缓肺功能下降，降低生活质量评分及呼吸困难评分。但在体重指数方面，三组患者治疗前、后体重指数变化不明显，差异无统计学意义（P〉0．05）。结论：法舒地尔联合舒利迭更能有效改善患者肺功能，提高患者的生存质量及改善预后。OBJECTIVE： To explore the effect of quality of life on Fasudil and Seretide in the treatment of chronic obstructive pulmonary disease （COPD） combined with pulmonary arterial hypertension （PAH） in the patients. METHODS ： The patients with COPD combined with PAH in our hospital from January 2013 to October 2014 were selected. The control group received conventional anti -inflammatory antispasmodic phlegm and other treatment. Fasudil was given in Fasudil group, on the basis of treatment in the control group. Fasudil and Seretide were given together in the combined treatment group. The difference of indexes of pulmonary func- tion, quality of life, dyspnea score and body mass index were compared in the three groups. RESULTS ： After treatment there were significant differences in pulmonary function, quality of life and dyspnea score in the three groups （P 〈 0. 05 ）. The curative effect in combined treatment group was more effetive than that in Fasudil group. There was no significant difference in the body mass index in the three group （P 〉 0.05）. CONCLUSION ： The treatment of Fasudil and Seretide can effectively improve lung function, life quality and the prognosis of the patient with COPD combined with PAH.
