摘  要:目的探讨甲状腺功能亢进症患者骨代谢生化指标、骨密度和白细胞介素-6改变情况。方法选取甲状腺功能亢进症患者78例为观察组,健康人群68例为对照组,观察两组血清游离T3(FT3)、游离T4(FT4)、血清人骨特异性碱性磷酸酶B(B-ALP)、尿脱氧吡啶啉(DPD)、血清骨钙素(BGP)、血清Ⅰ型前胶原氨基端前肽(PINP)和血清Ⅰ型胶原降解产物(ICTP)、血钙、24小时尿钙含量差异,L_(2-4)和股骨近端骨密度差异,血清白细胞介素-6含量和mRNA表达的不同。结果与对照组相比,观察组血清FT3、FT4、B-ALP、BGP、PINP、ICTP、DPD、24小时尿钙含量显著增加,L_(2-4)和股骨近端的骨密度测量值显著降低,血清IL-6含量和mRNA表达水平上升。结论甲状腺激素可直接参与加速骨转换及增加骨吸收,导致骨量损失严重,出现骨质疏松。Objective To investigate the biochemical indexes of bone metabolism,bone mineral density and IL-6changes in patients with hyperthyroidism.Method The serum levels of free triiodothyronine(FT3),free tetraiodothyronineT4(FT4).alkaline phosphatase B(B-ALP),DPD,BGP,blood I type collagen amino end propeptide(PINP)and blood type I collagen degradation products(ICTP),serum calcium,24 h urinary calcium,L(2-4) and femoral proximal bone density,the serum level of IL-6 and mRNA expression of IL-6 were detected and compared separately in case group and normal control group.Result The serum value of FT3,FT4,B-ALP,DPD,BGP,PINP,ICTP and 24 h urinary calcium in the patients with hyperthyroidism were significantly higher than in normal control proup.The bone mineral density of the proximal femur in the patients with hyperthyroidism was lower than in normal control group.The serum IL-6 level and the expression of mRNA in the patients with hyperthyroidism were significantly higher than in normal control group.Conclusion Thyroid hormone might speed up bone turnover directly with increasd bone resorption in patients with hyperthyroidism,which leads to the loss of bone mass,leading to osteoporosis.
