摘  要:目的探讨青春期多囊卵巢综合征(PCOS)的发生是否与特发性中枢性性早熟(ICPP)具有相关性。方法采用回顾性队列研究,选取2007年1月1日-2009年12月31日期间确诊的女性ICPP 288例为研究组,186例无性早熟病史的13~18岁且月经来潮3年以上女童为对照组,分析两组间青春期PCOS的发病率。结果两组女童之间在年龄、身高、体重方面差异有统计学意义(F值分别为34.359、21.08和7.31,均P<0.05),ICPP组高于对照组,而在初潮年龄方面,ICPP组早于对照组,差异有统计学意义(F=64.77,P<0.05)。ICPP组PCOS发病率6.60%,对照组PCOS发病率4.30%,2组PCOS发病率差异无统计学意义(χ~2=1.109,P>0.05)。Logistic逐步回归分析表明体重指数(BMI)及年龄与PCOS发病呈正相关。结论性早熟患者在青春期阶段发展为PCOS的风险并未增加,但仍需重视和长期随访。Objective To explore the correlation between pubertal polycystic ovary syndrome(PCOS) and idiopathic central precocious puberty(ICPP).Methods A retrospective cohort study was conducted,288 girls with ICCP were selected as study group,and 186 girls aged 13-18 years old,without ICCP history and menophania more than 3 years were selected as control group;the incidence rates of pubertal PCOS in the two groups were analyzed.Results There were statistically significant differences in age,height,and weight of girls between the two group(F=34.359,21.08,7.31,P0.05),the above-mentioned indexes in ICCP group were higher than those in control group;the age of menarche in ICCP group was statistically significantly earlier than that in control group(F=64.77,P〈0.05).The incidence rates of PCOS in ICCP group and control group were 6.60% and 4.30%,respectively,there was no statistically significant difference between the two groups(χ~2=1.109,P〉0.05).Logistic stepwise regression analysis showed that body mass index(BMI) was positively correlated with age and onset of PCOS.Conclusion The risk of PCOS doesn't increase in puberty among ICPP patients,but attention and long-term follow-up are still needed.
