中医手法联合中药熏洗治疗儿童肱骨髁上骨折术后肘关节功能恢复临床观察

摘　　要：目的:观察中医手法联合中药熏洗对儿童肱骨髁上骨折术后肘关节功能恢复的临床疗效。方法:将75例肱骨髁上骨折术后患儿随机分为观察组40例和对照组35例,对照组予肘关节牵引及肘关节功能锻炼治疗,观察组在对照组基础上予中医手法治疗及中药熏洗治疗,总疗程为4周,评估2组VAS评分及Mayo肘关节功能评分,并采用Flynn临床功能评定标准评价临床疗效。结果:经治疗,观察组优良率为92.50%,优于对照组的68.57%,差异有统计学意义(P<0.01)。疗程结束后2组患儿疼痛均缓解,肘关节活动度及稳定性提高,与治疗前相比VAS评分下降,肘关节功能评分上调,差异有统计学意义(P<0.01,P<0.05),观察组较对照组VAS评分及肘关节功能评分改善更加明显(P<0.05,P<0.01)。结论:中医手法联合中药熏洗治疗儿童肱骨髁上骨折术后,能有效缓解患儿疼痛症状,促进肘关节功能恢复。Objective： To observe the clinical effect of Chinese medicine manipulation combined with Chinese medicine fumigation on elbow function recovery after operation in children with supracondylar fracture of humerus. Methods： Selected75 patients with supracondylar fracture of humerus, and divided them into the observation group of 40 cases and the control group of 35 cases. The control group was given the treatment of elbow traction and elbow function exercise. The control group was treated with Chinese medicine manipulation combined with Chinese medicine fumigation based on the treatment of the control group. Total treatment lasted for four weeks. Evaluated visual analoguescale（VAS） and Mayo elbow function score of the two groups, and evaluated the clinical efficacy with Flynn clinical function evaluation criteria. Results： After treatment, the excellent and good rate of the observation group was 92.50%, higher than the 68.57% of the control group（P 〈 0.01）. After treatment, pain of patients in the two group were relieved, mobility and stability of elbow joint were increased. Compared with before treatment,VAS scores were decreased,elbow function scores were increased,the difference being significant（P 〈 0.01, P 〈 0.05）. The improvement of VAS scores and elbow joint function scores of the observation group were more significant than that of the control group（P 〈 0.05, P 〈 0.01）. Conclusion： The combination usage of Chinese medicine manipulation and Chinese medicine fumigation can relieve pain of patients effectively, and promote the recovery of elbow function.
