摘 要:目的介绍小儿足趾移植重建拇、手指的可行性及远期疗效。方法自2005年6月至2015年12月通过游离足趾移植重建小儿拇、手指11例16指,其中重建拇指8指、示指6指、中指2指。结果术后11例重建拇、手指均存活,足供区植皮部边缘坏死1例,换药后愈合。随访1-5年,重建指生长发育满意,较好地恢复了抓捏功能,两点分辨觉5-8mm。按中华医学会手外科学会拇、手指再造功能评定试用标准评价:优8例,良3例。供足行走正常,功能无影响。结论采用游离足趾移植重建小儿拇、手指缺损疗效满意。Objective To investigate the feasibility and long-term efficacy of reconstructing thumb and fingers with toe transplantation in children. Methods From June 2005 to December 2015, 11 cases (16 fingers) were reconstructed with toe transplantation. There were 8 thumbs, 6 index fingers, and 2 middle fingers. Results All the reconstructed fingers survived. Necrosis of skin grafts at the donor site was seen in 1 case, but healed after changing dressings. All the patients were followed-up from 1 to 5 years. The growth and development of the reconstructed fingers were satisfactory. The function of grip was regained. Two-point discrimination was 5 to 8 mm. According to the functional assessment criteria for thumb and finger reconstruction issued by the Hand Surgery Society of the Chinese Medical Association, the outcomes were excellent in 8 cases and good in 3 cases. There was no abnormal gait and pain in the donor foot. The function of the foot was not impaired. Conclusion The outcomes of toe transplantation in reconstruction of thumb and finger defects in children were satisfactory.
