益肾冲剂与血必净防治对比剂肾功能损害临床对照研究

摘　　要：目的:探讨益肾冲剂能否减低急性冠脉综合征患者PCI术后对比剂肾病(CIN)的发生率。方法:选择急性冠脉综合征患者300例,随机分为3组。对照组常规给予抗血小板、抗凝及水化等治疗。益肾冲剂组在对照组的用药基础上于术前1 d至术后7d服用益肾冲剂。血必净组患者于术前1 d至术后7d静脉点滴血必净(天津红日药业有限公司),50ml/Qd。所有患者分别于术前1 d至术后7d测定SCr、尿微量白蛋白。结果:益肾冲剂组与血必净组冠状动脉造影术后1日及术后2日冠心病患者血清Scr、尿微量白蛋白与对照组分别相比较,差异有统计学意义,P<0.05。益肾冲剂组与血必净组冠状动脉造影术后1日及术后2日冠心病患者血清Scr、尿微量白蛋白与对照组分别相比较,差异无统计学意义,P>0.05。结论:对于对比剂引发的急性肾损伤,在常规水化治疗的基础上加用益肾冲剂或血必净注射液对冠心病患者行冠状动脉造影术后的肾功能具有保护作用,可以更有效地预防造影剂肾病的发生。Objective： to study the kidney granule can reduce the patients with acute coronary syndrome incidence of contrast medium nephropathy （CIN） after PCI. Method： choose 300 cases of patients with acute coronary syndrome （acs）, were randomly divided into three groups. Such as the control group conventional antiplatelet and anticoagulant and hydration treatment. Kidney granule group in the control group of drugs based on preoperative 1 d to 7 d after taking kidney granule. Blood will net group of patients in the preoperative 1 d to 7 d after intravenous drop of blood will net red sun pharmaceutical co., LTD. （tianjin）, 50 ml/Qd. All of the patients in preoperative 1 d to 7 d after determination of SCr, urine trace albumin. Results： kidney granule group and the blood shall be net of coronary arteriography and postoperative after 1,,2, coronary heart disease （CHD） patients＇ serum levels of Scr, compared with the control group respectively, urine trace albumin difference was statistically significant, P 〈 0.05）. Kidney granule group and the blood shall be net of coronary arteriography and postoperative after 1, 2, coronary heart disease patients serum levels of Scr, urine trace albumin compared with the control group respectively, there was no statistically significant difference, P 〉 0.05）. Con^clusion： the contrast medium caused by acute kidney injury, on the basis of routine hydration treatment with reasons of granule or blood will net injection line of coronary angiography in patients with coronary heart disease after renal protective, can more effectively prevent the happening of the contrast nephropathy.
