多层螺旋CT血管造影在咯血介入栓塞前的评估价值

摘　　要：目的:探讨多层螺旋CT血管造影在咯血患者介入栓塞前的评估价值。方法:回顾性分析28例同时行术前MSCTA和DSA检查的咯血患者影像学资料,观察MSCTA对咯血责任血管的显示效果,以及对介入栓塞的指导价值。结果:MSCTA共检出27例患者40支咯血责任血管,DSA检出28例43支咯血责任血管,术前MSCTA对咯血责任血管显示的敏感性为93%,特异性为100%,两者对咯血责任血管的检出率无明显差异(P>0.05)。27例患者在MSCTA指引下均能顺利选择性插管、成功栓塞。结论:介入术前MSCTA可提供咯血相关动脉准确、详细的影像解剖信息,指导介入栓塞治疗。Objective： To investigate the value ofMSCT angiography in preoperative endovascular embolization of hemoptysis. Methods： 28 cases of hemoptysis with MSCTA and DSA examination were selected. Diagnostic performances of MSCTA in depicting arteries were assessed. Results： 40 feeding vessels were detected by MSCTA in 27 cases and by DSA in 28 cases. The sensitivity, specificity was 93% and 100%. There was no significant difference between preoperative MSCTA and DSA in dedecting feediong vessels （P〉0.05）. 27 cases had selective catheterization and successful embolization. For 3 months＇ follow-up, 2cases with recurrent massive hemoptysis were found. Conclusion： Preoperative interventional MSCTA can provide detailed information of correlate artery in embolization therapy.
