摘  要:目的探讨米力农联合机械通气治疗新生儿持续肺动脉高压(PPHN)的疗效及有效护理措施。方法选择2013年1月至2016年2月期间我院新生儿科收治的56例新生儿持续肺动脉高压患儿,根据随机数字表法随机分为对照组和观察组,每组28例,对照组予常频机械通气治疗,观察组在此基础上联合米力农治疗,疗程3 d。比较两组患儿治疗前后的肺动脉收缩压(PASP)的变化、机械通气时间及临床疗效,总结有效护理措施。结果治疗后观察组与对照组患儿PASP分别为(30.5±12.3)mm Hg和(39.2±14.2)mm Hg,均较治疗前的(65.3±19.6)mm Hg和(66.5±18.7)mm Hg明显降低,且观察组低于对照组,差异均有统计学意义(P<0.05);观察组患儿机械通气时间为(88.7±22.8)h,明显短于对照组的(101.6±24.1)h,差异有统计学意义(P<0.05);观察组患儿治疗总有效率为85.71%,明显高于对照组的64.29%,差异有统计学意义(P<0.05)。结论米力农联合机械通气治疗PPHN可进一步降低PASP、缩短机械通气治疗时间进而提高临床疗效;有效的护理措施则可保证治疗顺利、有效地进行,减少治疗相关并发症,对改善患儿的预后有利。Objective To investigate the clinical effect and effective care measures of milrinone combined with mechanical ventilation in treatment of persistent pulmonary hypertension of the newborn(PPHN). Methods A total of 56 cases of PPHN in Department of Neonatology in our hospital from Jan. 2013 to Feb. 2016 were selected, which were randomly divided into the control group and the observation group according to the random number table method,with 28 cases in each group. The control group was treated with normal frequency mechanical ventilation, and the observation group received milrinone treatment based on the control group, with course of treatment for three days. The pulmonary artery systolic pressure(PASP) changes, duration of mechanical ventilation and its clinical efficacy before and after treatment were compared, and the effective care measures were summarized. Results After treatment, the PASP in observation group and control group were respectively(30.5±12.3) mm Hg and(39.2±14.2) mm Hg, which were significantly lower than those before treatment of(65.3±19.6) mm Hg and(66.5±18.7) mm Hg, the PASP in observation group were significantly lower than that in the control group(P0.05). The mechanical ventilation time in observation group was(88.7±22.8) h, which was significantly shorter than that in control group of(101.6±24.1) h, and the difference was statistically significant(P0.05). The total effective rate in the observation group was 85.71%, which was significantly higher than that in control group of 64.29%(P0.05). Conclusion Milrinone combined with mechanical ventilation in treatment of PPHN can further reduce PASP, shorten mechanical ventilation time and improve clinical curative effect. Effective nursing measures can ensure a smooth and effective treatment, reduce the treatment related complications, and improve the prognosis.
