早期可疑肺部感染患者行CT联合X线检查的临床研究

摘　　要：目的：探究早期可疑肺部感染患者行CT联合X线检查的临床价值。方法：病例来源于某院2012年1月至2014年7月收治的疑为肺部感染患者110例,对所有病患均施行胸部CT与X线片检查,同期选取体检健康者110例作为对照,比较X线检查与CT联合X线检查对诊断肺部感染的情况及病变的CT影像特征。结果：CT联合X线胸片检查对肺部感染的诊断率（92.73%）和诊断阳性率（86.36%）分别与单纯X线胸片（66.36%,51.82%）比较差异显著,具有统计学意义（χ^2=23.49,χ^2=30.74,P〈0.01）;与健康体检者相比,诊断为肺部感染患者的CT与X线片显像可观察到肺部阴影的形态、数量、密度、范围及其分布区域均具有明显变化。结论：X线胸片检查具有价格低廉、对患者的辐射稍少、在各大小医院普及率高可作为筛查肺部可疑感染和患者出院后随访的首选检查手段;胸部CT扫描的特异性与敏感性均较X线片优良,针对有明显临床症状但X线片未见异常者可施行CT检查,CT联合X线片检查使早期可疑肺部感染的确诊率大幅提高。Objective To explore theclinical value of combined X-ray and CT examinations for early stage of suspected pulmonary infection. Methods Totally 110 patients with suspected pulmonary infection from January 2012 to July 2014 underwent chest X-ray and CT examinations, and there were 110 healthy physical examinees were enrolled in a control group. X-ray and CT examinations were compared when used to diagnose pulmonary infection, and CT features of pulmonary infection were also discussed. Results Combined X-ray and CT examinations hadthe diagnostic rate and positive rate being92.73% and 86.36% respectively, which were significantly different from those（66.36% and 51.82% respectively） of X-ray examination（ χ^2=23.49, χ^2=30.74, P 0.01）. The pulmonary infection patient had the lung shadow changed significantly in shape, number, density, range and distribution area when compared with the healthy examinee. Conclusion X-ray chest examination gains advantages in low cost, few radiation and high spreading rate, and thus can be the preferred choice for screening of suspected pulmonary infection and post-hospital followup; while CT scan behaves better than X-ray examination in specificity and sensitivity, and can be used in case overt clinical symptoms occur but X-ray examination finds no abnormality. Combined CT and X-ray examinations enhance the diagnosis rate of suspected pulmonary infection at early stage greatly.
