摘  要:目的:利用医院影像归档和通信系统(picture archiving and communication systems,PACS)对CT重复检查情况及辐射剂量进行分析,探讨CT扫描累积辐射剂量的影响因素及减少医疗辐射的方法。方法:收集2014年9月至2015年9月接受2次及2次以上CT检查的患者,分析其重复检查频次、重复检查原因及CT辐射剂量。结果:1 a中有3 241名患者接受了2次及2次以上的CT重复检查,占总检查人数的7.30%。重复检查2~4次最多,占64.05%;重复检查部位头部、胸部较多,重复检查患者多数为60岁以上;重复检查原因恶性肿瘤占首位,其次分别为胸部病变、脑血管病以及外伤。随着CT重复检查次数的增加,辐射剂量也明显上升。结论:医院有必要对肿瘤、外伤、脑血管病等常见疾病进行CT检查频率及累积剂量的监测,对医务人员进行辐射安全教育,避免不恰当的CT检查,减少医疗辐射。Objective To analyze the repeated CT scans and accumulated radiation doses with PACS, and to explore the affecting factors of accumulated radiation doses of CT scans and the methods for reducing medical radiation. Methods The patients accepted two or more times of CT scans were collected from September 2014 to September 2015, who were analyzed on the frequencies and reasons for repeated CT scans as well as CT radiation dose. Results There were 3 241 patients received two or more times of CT examination in a year, accounting for 7.30%. There were 64.05% of the patients undergoing two to four times of scans. The cases going through head and chest examinations were most older than 60 years old. The most common reason for repeated scan was cancer, and other reasons included chest lesion, cerebrovascular disease and trauma. Increased times of scans resulted in raised radiation dose. Conclusion CT scan frequency and accumulated dose have to be monitored for tumor,trauma, cerevrovascular disease and etc, and medical personnel should be educated on radiation safety to avoid unnecessary CT scan and decrease medical radiation.
